脐带绕颈与足月新生儿视网膜出血的相关性研究

摘　　要：目的 探究脐带绕颈（NC）与足月新生儿视网膜出血（RH）的关系。方法 收集2015年2-11月在本院出生后5d内接受RetCamⅢ眼底检查的1 489例足月新生儿资料,除外全身系统性疾病和其他眼部疾病。将受检新生儿分为NC组和对照组,分析NC与新生儿RH的关系。结果 1489例受检新生儿共有RH251（16.9%）例。NC组RH的发病率为21.3%,其中绕颈1周RH的发病率为23.7%,绕颈2周RH的发病率为25.6%,绕颈3周及以上RH的发病率为5.3%;对照组RH的发病率为15.2%。NC组RH的发病率高于对照组,差异有统计学意义（P〈0.05）。结论 NC是新生儿RH的危险因素,应该引起相关临床医师的重视。Objective To study the relationship between nuchal cord （NC） and retinal hemorrhages （RH） in healthy full-term newborns. Methods A total of 1 489 full-term newborns with gestational age more than 37 weeks born in the Fourth Hospital of Shijiazhuang between February 2015 and November 2015 were included in this study. Newborns with severe systemic diseases or any other eye diseases were excluded. Newborns were divided into NC group and the control group. Eye examinations with RetCam]lI within 5 days of birth were performed in all newborns. The relationship between NC and RH in full-term infants was analyzed with statistic analysis. Results In all of the 1 489 newborns, RH were found in 251 cases （ 16.9% ） . There were 88 cases （21.3%） of newborns having RH in the NC group. The incidences of RH in newborns with NC for single loop, double loops, and multiple loops were 23.7% ,25.6% and 5.3 % respectively,whereas the incidence of RH in the control group was 15.2%. The significantly higher prevalence of RH was observed in NC group. Conclusions NC was found to be a potential risk factor for the development of newborn RH in full-term infants. The importance of screening infants with NC should be recognized by relevant physicians. （ Chin J Ophthalmol and Otorhinolaryngo1,2016 ,16 ：399-401 ）
