摘　　要：目的检测弥漫性大B细胞性淋巴瘤(DLBCL)中EB病毒与p100/p52蛋白的表达,进一步了解EBV阳性的DLBCL发病机制及临床病理意义,探讨DLBCL中EBV与p100/p52蛋白表达的相关性。方法应用原位杂交检测EBV编码mRNA(EBER-1)的表达;运用免疫组化方法检测LMP1及p100/p52蛋白的表达。结果 190例DLBCL中EBV(+)24例(12.6%),发病年龄>40岁,平均70.04岁。其中,汉族发病高峰年龄>70岁,维族>60岁,汉族患者发病年龄略高于维族。DLBCL中EBV阳性与阴性组进行对比,IPI和LDH差异显著(P<0.05)。24例EBV(+)中22例p100/p52蛋白(+),p100/p52蛋白与EB病毒之间存在明显相关性(P<0.05)。结论 p100/p52蛋白在EBV阳性DLBCL中高表达,参与DLBCL的发生、发展并促进肿瘤的生长;因存在相关性,则抑制剂可作为选择性治疗方式。NF-κB过表达是不良的预后标记,p100/p52也许能用于预测预后并在治疗性干预措施中成为一个潜在靶点。Objective To detect Epstein-Barr virus in the diffuse large B cell lymphoma and p100/p52 protein expression for further understanding of the EBV positive DLBCL pathogenesis and the clinical pathological features, and to explore the correlation between EBV and p100/p52 protein expression. Methods In situ hybridization detection of EBV encoded mRNA expression （EBER-1） was carried out and the immunohistochemical staining for LMP1 and p100/p52 proteins was performed using EnVISion method. Results EBV positivity was detected in 24 of 190 patients with DLBCL （ 12. 63% ）. The EBV positive cases were older than the age of 40, with average age of 70. 04 years, in which the peak age was 70 - in Han nationality, and 60 - years in Uighur, and Han patients with onset age were slightly higher than the Uyghur. Compared EBV positive with negative groups, both IPI and LDH were statistically significant （ P 〈 0. 05 ）. 22 of the 24 eases of EBV positive DLBCL presented p100/p52 protein expression, and p100/p52 proteins were significantly associated with Epstein- Barr virus （P 〈 0. 05）. Conclusion In EBV positive DLBCL, p100/p52 protein high expression may participate in the pathogenesis of DLBCL and promote the occurrence of tumor. Because there is relationship between them, the inhibitor can be used as a selective treatment. The NF-KB overexpression is an independent poor prognostic marker, and p100/p52 may be used to predict the prognosis and therapeutic interventions, which is a potential target.
