腹腔镜Dixon直肠癌根治术与经腹Dixon直肠癌根治术的对比分析

摘　　要：目的:探讨腹腔镜Dixon直肠癌根治术与经腹Dixon直肠癌根治术治疗效果及术后恢复。方法:回顾性分析近3年244例Dixon直肠癌根治术的临床资料,其中行腹腔镜术107例,与同期具有可比性137例开腹直肠癌根治术进行比较。结果:两组手术时间无明显统计学差异(P>0.05),腹腔镜组术中出血及术后首次肛门排气时间均优于传统经腹组(P>0.05);术中摘除淋巴结个数及不同浸润深度的淋巴结清扫数目两组无统计学差异(P>0.05)。腹腔镜组直肠癌根治术术后吻合口瘘发生率、伤口感染率、黏连性梗阻发生率较经腹组降低(P>0.05);腹腔镜组直肠癌性功能和排尿功能保护随访满意度高(P>0.05)。结论:腹腔镜直肠癌根治术出血少,术后肠功能恢复快,术后吻合口漏、伤口感染、黏连性肠梗阻发生率低,具有良好的近期效果。Objective：To investigate the effect and postoperative recovery of laparoscope radical resection for rectal cancer and open radical resection for rectal cancer.Methods：The clinical data of 244 cases of patients with radical resection of rectal cancer were retrospectively analyzed,including 107 cases of laparoscope surgery.Compared with the 137 cases of patients received open radical resection for rectal cancer in the same period.Results：There was no significant difference in operation time between the two groups（P〉0.05）.The amount of bleeding and the first time anal exhaust of laparoscope group were superior to that of traditional group（P〈0.05）;There was no significant difference in the number of removed lymph nodes and the cleaning number of lymph nodes with different infiltration depth between the two groups（P〉0.05）.The incidence of anastomotic leakage,wound infection rate,and the incidence of adhesive obstruction of laparoscope group were lower than those of traditional group（P〈0.05）.The follow-up satisfaction of rectal cancer patients＇ sex function and micturition function protection in laparoscope group were high（P〈0.05）.Conclution：Laparoscope radical resection for rectal cancer has advantages of less bleeding and postoperative intestinal function quick recovery,but the postoperative anastomotic leakage,wound infection,and the incidence of sticky even intestinal obstruction werelow,which has a good recent effect.
