保乳联合前哨淋巴结活检术对乳腺癌的治疗价值评析

摘　　要：目的研究保乳联合前哨淋巴结活检术对乳腺癌的治疗价值。方法 83例乳腺癌患者,随机分为传统组(41例)和研究组(42例)。传统组行传统全乳切除手术联合腋窝淋巴结清扫,研究组行保乳联合前哨淋巴结活检术。比较两组患者乳房美容效果优良率、术后远处转移率、局部复发率、并发症率、3年生存率,以及手术操作时间、平均失血量、引流量、拔管时间的差异。结果研究组乳房美容效果优良率为95.24%,高于传统组的75.61%(P<0.05),研究组并发症发生率为2.38%,低于传统组的34.15%(P<0.05);两组患者远处转移率、局部复发率、3年生存率比较差异无统计学意义(P>0.05);研究组较传统组手术操作时间、失血量、引流量、拔管时间明显更少(P<0.05)。结论保乳联合前哨淋巴结活检术对乳腺癌的治疗价值高,手术简便,创伤低,出血少,引流量少,可缩短拔管时间,减少并发症的发生,提高美容效果,值得推广。Objective To research treatment value by breast conserving surgery and sentinel lymph node biopsy for breast cancer. Methods A total of 83 patients with breast cancer were randomly divided into traditional group（41 cases） and research group（42 cases）. The traditional group received traditional mastectomy combined with axillary lymph node dissection, and the research group received breast conserving surgery and sentinel lymph node biopsy. Comparison was made on breast cosmetic effect good rate, postoperative distant metastasis rate, local recurrence rate, incidence of complications, 3-year survival rate, operation time, mean bleeding volume, drainage volume and extubation time between the two groups. Results The research group had higher breast cosmetic effect good rate as 95.24% than 75.61% of the traditional group（P〈0.05）, and its incidence of complications was lower as 2.38% than 34.15% of the traditional group（P〈0.05）. There was no statistically significant difference of distant metastasis rate, local recurrence rate and 3-year survival rate between the two groups（P〉0.05）. The research group had obviously less operation time, bleeding volume, drainage volume and extubation time（P〈0.05）. Conclusion Combination of breast conserving surgery and sentinel lymph node biopsy contains high treatment value for breast cancer, along with simple operation, low trauma, little bleeding and drainage. This method can shorten extubation time, reduce complications and improve cosmetic effect, and it is worth promoting.
