摘 要:目的应用微RNA(miRNA)高通量测序技术探索缺血性脑卒中二级预防中复发患者与不复发患者初次发病后血浆miRNA表达谱差异,并预测差异表达的miRNA的靶基因及调节通路。方法收集800例初发缺血性脑卒中后接受规范抗血小板二级预防的汉族人的临床资料并进行为期1年的随访。29例随访中再次出现缺血性脑卒中者入选复发组;根据危险因素1∶1匹配对照的原则,在随访对象中入选29例1年内无再发脑卒中患者为对照组。通过Illumina高通量测序分析两组患者血浆miRNA表达谱差异,根据测序结果及文献报道,从中筛选1个组间表达差异较大的且与血栓形成相关的miRNA用实时荧光定量法(qRT-PCT)进一步验证。通过mireap、miranda、targetscan三个软件进行靶基因预测。结果组间miR-150-5p的表达差异有统计学意义,其靶基因为ENST00000221264,编码纤溶蛋白酶原激活物。结论 miR-150-5p与缺血性脑卒中患者二级预防治疗的中期(1年)预后有关,它参与到脑血栓形成的生理病理过程中,特异的miRNA可望成为评估缺血性脑卒中二级预防中评估抗血小板治疗效应的新的生物标志物。Objective To investigate the correlation between plasma miRNA and the prognosis of ischemic stroke in secondary prevention by using miRNA illumina sequencing technique and to predict the gene target and the molecular pathway of specific miRNA. Methods A total of 800 Han Chinese patients who suffered from primary ischemic stroke and received antiplatelet as secondary prevention were sequentially recruited to the study and followed for up to one year. Plasma samples were collected and then persevered at the time of their primary stroke. Twenty - nine of them suffered from a secondary stoke within one year were enrolled as recurrent group. According to the 1 : 1 pairing principle of risk factors, 29 patients free from a secondary ischemic attack were selected as controlling group. Plasma miRNA were analyzed by illu- mina sequencing. According to the result of illunima sequencing and previous report, miR- 150 was selected to undergo qRT - PCR for further confirmation. Significant expression difference was shown in qRT - PCR of miR - 150, Which targe- ted at gene ENST00000221264 encoding plasminogen activator. Results We found that miR - 150 was closely correlated to the mid - term (within 1 year) prognosis of secondary prevention of ischemic stroke after a primary attack, which was also found to associate with the pathogenesis of coagulation. Conclusion The specific miRNA may have the: potential to be a novel biomarker to assess the efficacy of antiplatelet therapy in secondary prevention of ischemic stroke.
