腹腔镜与开腹直肠前切除术的近期疗效对比分析

摘　　要：目的:对比分析腹腔镜与开腹直肠前切除术治疗直肠肿瘤的近期临床效果。方法:选取2014年5月至2015年10月收治的105例直肠肿瘤患者,随机分为腹腔镜组(行腹腔镜手术,n=52)与开腹组(行开腹手术,n=53),对比两组患者的手术相关指标(包括手术时间、术中出血量、切口长度)、术后恢复指标(包括术后下床活动时间、拔除引流管时间、第一次排气时间、住院时间)、肿瘤根治性指标(淋巴结清扫数量、肿瘤远端切缘、切除肿瘤标本长度)。结果:两组患者手术时间、术中出血量、切口长度差异有统计学意义(P<0.05);两组肿瘤根治性指标差异无统计学意义(P>0.05);腹腔镜组术后下床活动时间、进流质食物时间、第一次排气时间、拔除引流管时间、住院时间均短于开腹组(P<0.05)。结论:直肠肿瘤患者行腹腔镜手术是安全、可行的,与传统开腹手术相比,具有更好的近期临床疗效。Objective： To compare and analyze the recent clinical effect of laparoscopic and open Dixon operation in the treatment of rectal tumor.Methods： From May 2014 to Oct.2015,105 patients with rectal tumor were selected and randomly divided into the laparoscopic group（ n = 52） and the laparotomy group（ n = 53）,the patients of the laparoscopic group were treated with laparoscopic surgery,the patients of the laparotomy group were treated by laparotomy.Operative indicators（ operative time,intraoperative blood loss,incision length）,postoperative recovery indexes（ postoperative ambulation time,time of drainage tube removal,time to first flatus,hospital stay） and tumor radical related indexes（ harvested lymph node,distal edge to the tumor,length of the specimen） were compared between the two groups.Results： The operative time,intraoperative blood loss and incision length of two groups were significantly different（ P 〈0.05）.The cancer radical related indicators were not statistically significantly different（ P 〉 0.05）.The postoperative ambulation time,first time of intaking liquid food,time to first flatus,time of drainage tube removal,hospital stay of laparoscopic group were significantly shorter than those of laparotomy group（ P 〈 0.05）.Conclusions： Laparoscopic surgery is safe and feasible in the treatment of rectal tumor,compared with the traditional open operation,laparoscopy has better clinical curative effect in the near future.
