摘 要:目的探讨内耳HRCT标准化测量对微小内耳畸形的先天性感音神经性耳聋(SNHL)患儿与正常儿童内耳解剖的价值。方法对32例无明显影像学内耳畸形的先天性SNHL患儿和76例正常儿童行颞骨HRCT检查,标准化测量上半规管、外半规管及后半规管的骨管及骨岛的宽度、耳蜗的最大高度和宽度、前庭的最大宽度和长度等。采用SPSS13.0统计软件的t检验比较实验组与正常组测量数据差异。结果两组数据在外半规管骨岛的宽度(lateral semicircular canal,LSCC),正常组:(3.06±0.36)mm、SNHL组:(3.03±0.03)mm、耳蜗的最大高度,正常:(4.46±0.11)mm、SNHL组:(4.51±0.09)、上半规管骨岛宽度(superior semicircular canal,SSCC),正常:(4.54±0.39)mm、SNHL组:(4.32±0.45)mm之间有明显的差异(P<0.05)。SNHL患儿的外半规管骨岛宽度、上半规管骨岛宽度测量值均小于正常儿童(3.03±0.03)mm vs(3.06±0.36)mm,(4.32±0.45)mm vs(4.54±0.39)mm(Pp<0.05);耳蜗最大高度测量值大于正常儿童(4.51±0.09)mm vs(4.46±0.11)mm(P<0.05)。结论标准化HRCT测量上半规管、外半规管骨岛宽度及耳蜗最大高度可对先天性SNHL进行诊断。Objective The purpose of this study is to use the HRCT standard measurement methods of the inner ear to observe the inner ear malformations in patients suffering from congenital deafness and to explore its diagnostic value in the diagnosis of congenital sensorineural hearing loss. Methods The experimental group includes a total of 50 patients with congenital sensor neural hearing loss of the inner ear, while the control group included 152 normal inner ear scan image. The measurements of this study were in the axial CT scanned images to obtain the measurement data, Two senior radiologists used the double blind means to measure all the image data, including the width, highness of SSCC, LSCC, PSCC (posterior semicircular canal ), cochlea and vesti- bule. The corresponding data for the control groups and experimental groups were compared using the "t" test. Results The axial CT images of the temporal can display the anatomic structure primeply. There were significant differences in the measurements of the bony island width of the superior semicircular canal ( normal group : (4.54 ± 0. 39) mm, SNHL group : (4.32 ±0.45 ) mm, bony island width of the lateral semicircular canal normal group : (3.06 ±0.36) mm, SNHL group : ( 3.03 ± 0.03 ) mm and maximal height of cochlea ( normal group : (4.46 ±0.11 ) ram, SNHL group : (4.51 ± 0.09 ) between two groups ( P 〈 0.05 ). Conclusion To measure the maximum height of the cochlea, the width of the semicircular canal and lateral semicircular canal bone island determine some subtle deformities.
