超声密度定量分析技术在均匀性脂肪肝的诊断价值

摘　　要：目的探讨超声密度定量分析技术(AD)在均匀性脂肪肝诊断中的应用价值。方法将收治的均匀性脂肪肝患者45例,按病情严重程度分为轻度脂肪肝16例、中度脂肪肝19例和重度脂肪肝10例。另收集45例健康体检者。超声密度定量测定两组受试者肝脏近、远场的背向散射积分(IBS),并进行对比分析。结果轻、中、重度脂肪肝患者近场和中场的IBS较对照组较明显增高(P<0.05),轻、中度脂肪肝患者远场的IBS与对照组也有差异(P<0.05),而重度脂肪肝患者远场的IBS与对照组的差异无统计学意义(P>0.05)。脂肪肝组近、中、远场IBS值呈现明显的递减趋势,并且随着病情的加重,其近场与中场IBS的值发生明显升高。结论超声密度定量技术对于均匀性脂肪肝可进行量化分析,对脂肪肝的分级和疗效判断提供客观依据,具有较高的临床应用价值。Objective To explore the diagnostic value of the acoustic densitometry （AD） in homogeneous fatty liver. Methods 45 patients with uniformity of treating fatty liver, according to the illness severity, were divided into 3 groups, 16 cases in mild fatty liver group, 10 cases in moderate fatty liver group and 10 cases in severe fatty liver group. And 45 cases of healthy vol- unteers were collected for control group. The acoustic densitometry was used to measure the near and far field of the backscattering integral （IBS） in the two groups of subjects, and the results of the survey were conducted in-depth and detailed analysis. Results The near-field IBS in light, medium and severe fatty liver patients were higher than that of control group obviously （ P 〈 0.05 ） , the far field IBS of patients with mild and moderate fatty liver was also higher than that of the control （ P 〈 0.05 ） , and the far field IBS of patients with severe fatty liver had no statistical significant difference than that of control group （ P 〉 0.05 ）. The near, middle and far field IBS value in fatty liver group present obvious decreasing trend, and as the illness was aggravating, the IBS value of near field and midfield increased significantly. Conclusion The acoustic densitometry technique can be quantified analysis for homogeneous fatty liver, and provide objective basis for fatty liver disease classification and curative effect judgment, so it could have high clinical value.
