摘　　要：目的:对比经典与非经典抗精神病药物对精神分裂症患者的疗效、社会功能及安全性影响。方法:选取460例服用药物维持治疗的精神分裂症患者,进行为期5年的随访观察,随访结束时将剩余的205例患者根据所用抗精神病药物的种类分为经典药物组95例和非经典药物组110例,并应用阳性与阴性症状量表(PANSS)、个人和社会功能量表(PSP)及副反应量表(TESS)对两组患者基线期、随访第2年和第5年的疗效、社会功能及药物副反应进行评估。结果:随访2年及5年时两组PANSS总分及大部分因子分较基线期有下降,且差异有统计学意义,但同期两组间对比显示仅阴性因子、焦虑/抑郁、认知因子差异有统计学意义。对比基线期,随访2年及5年时两组PSP分均有升高,但仅非经典组差异有统计学意义。结论:非经典药物在改善精神分裂症患者的阴性症状、情感症状、认知功能及社会功能方面优于经典药物,且安全性更高。Objective To compare the effects of typical and atypical antipsychotics on the efficacy, social function and safety of schizophrenia. Methods Four hundred and sixty cases of schizophrenia treated with drug maintenance treatment were chosen to complete a 5-year follow-up. Two hundred and five patients completed the follow-up and were divided into the typical drug group with 95 cases and the atypical drug group with 110 cases according to the types of antipsychotic drugs used. During the baseline period and in the second and the fifth year of follow-up, the efficacy, social function and safety of two groups were assessed with the positive and negative symptoms scale （PANSS）, personal and social performance scale （PSP） and treatment emergent symptom scale （TESS） ,respectively. Results Compared to the baseline period, the PANSS scores in the two groups were significantly lower than those in the second year and the fifth year of follow-up. The negative scores, the affective symptoms and the cognitive factor of PANSS were significantly different between the two groups. PSP scores in the second year and the fifth year were significantly higher than that in baseline period in the atypical group. Conclusion Compared to the typical antipsychotics, atypical antipsychotics can improve the negative symptoms, emotional symptoms, cognitive function and social function of schizophrenia patients, with more safety.
