中国女性子宫内膜癌危险因素的Meta分析

摘　　要：目的评价中国女性子宫内膜癌危险因素的作用,探讨子宫内膜癌的病因。方法运用Meta分析的方法对2000年‐2015年在国内外公开发表的有关中国女性子宫内膜癌危险因素的18篇病例对照研究文献进行定量综合分析。结果各因素合并OR值及95%CI分别为：子宫内膜癌家族史3.28（1.72~6.24）,高血压3.02（1.92~4.76）,体重超重或肥胖2.37（1.60~3.49）,糖尿病2.35（1.62~3.41）,恶性肿瘤家族史2.17（1.46~3.24）,宫内节育器0.43（0.21~0.88）,生育史0.51（0.26~1.01）,口服避孕药史0.55（0.38~0.79）,饮酒史0.70（0.49~1.00）,经常锻炼0.81（0.72~0.90）,绝经状态0.87（0.79~0.94）,哺乳史0.82（0.64~1.04）,激素替代疗法（HRT）史0.98（0.76~1.26）,吸烟史1.04（0.83~1.31）,饮茶史0.94（0.83~1.06）。结论子宫内膜癌家族史、高血压、体重超重或肥胖、糖尿病及恶性肿瘤家族史是子宫内膜癌的危险因素,宫内节育器使用史、生育史、口服避孕药史、饮酒史、经常锻炼及绝经状态是子宫内膜癌的保护因素。【Objective】To explore the pathogenesis of endometrial cancer through appraising the main risk factors related to the incidence of endometrial cancer among Chinese women.【Methods】The results from 18 case-control studies on risk factors of endometrial cancer from 2000 to 2015 were analyzed by Meta-analysis.【Results】The pooled odds ratio（OR） and 95% confi dence interval（CI） were： family history of endometrial cancer 3.28（1.72~6.24）, hypertension 3.02（1.92~4.76）, overweight 2.37（1.60~3.49）, diabetes 2.35（1.62~3.41）, family history of carcinoma 2.17（1.46~3.24）, intrauterine device（IUD） use 0.43（0.21~0.88）, fertility history 0.51（0.26~1.01）, oral contraceptive use 0.55（0.38~0.79）, alcohol consumption 0.70（0.49~1.00）, regularly exercise 0.81（0.72~0.90）, menopausal status 0.87（0.79~0.94）, breast-feeding 0.82（0.64~1.04）, hormone replacement therapy（HRT） 0.98（0.76~1.26）, cigarette smoking 1.04（0.83~1.31）, tea drinking 0.94（0.83~1.06）.【Conclusion】The risk factors of endometrial cancer among Chinese women were family history of endometrial cancer, hypertension, overweight, diabetes, and family history of carcinoma, while the protective factors were IUD use, fertility history, oral contraceptive use, alcohol consumption, regularly exercise, and menopause.
