摘 要:目的:检测缺血性脑卒中(ischemic stroke,IS)患者龈下菌斑中牙周致病菌的分布情况,及其与慢性牙周炎(chronic periodontitis,CP)病变程度之间的关系。方法:收集49例IS合并CP患者,40例IS无CP患者及45例单纯CP患者的龈下菌斑,提取细菌DNA,采取聚合酶链式反应(PCR)检测牙龈卟啉单胞菌(Porphyromonas gingivalis,Pg)、福赛坦菌(Tannerella forsythia,Tf)、具核梭杆菌(Fusobacterium nucleatum,Fn)、中间普氏菌(Prevotella intermedia,Pi),并比较4种微生物在IS合并CP,IS无CP患者及单纯CP患者龈下菌斑中的检出率。结果:IS合并CP组细菌检测出率为:Pg75.51%,Tf97.96%,Fn91.83%,Pi 67.35%,Tf和Fn的检出率显著高于Pg、Pi 。IS无CP组细菌检测出率为:Pg 37.50%,Tf 32.50%,Fn27.50%,Pi 25.00%。CP组细菌检出率为:Pg73.33%,Tf91.11%,Fn 86.67%,Pi 66.67%。IS合并CP组牙周致病菌检出率高于CP组,但差别无统计学意义(P>0.05),IS合并轻、中、重CP组Fn的检出率分别为75.00%,95.83%,100%,三者差异有统计学意义。结论:IS合并CP组及CP组的龈下菌斑中4种牙周可疑致病菌的分布无明显差别。IS合并CP组Fn的检出率随慢性牙周炎病变程度的加重而增加。Objective:To investigate the prevalence of periodontal pathogens in the subgingival plaque of patients with ischemic stroke,and the relationship between the distribution of periodontal pathogens and various degrees of chronic periodontitis.Methods:49 subgingival plaque samples of patients with IS and CP,40 subgingival plaque samples of patients with IS without CP,and 45 subgingival plaque samples of patients with CP were collected and their bacteria DNA was extracted.Four periodontal pathogens including Porphyromonas gingivalis(Pg),Tannerella forsythia(Tf),Fusobacteriu mnucleatum(Fn),Prevotella intermedia(Pi) were examined using the polymerase chain reaction(PCR)technique.Results:In the IS combined with CP group,the rates of periodontal bacteria were Pg 75.51%,Tf 97.96%,Fn91.83%,Pi 67.35%.In the IS without CP group the rates of periodontal bacteria were Pg 37.50%,Tf 32.50%,Fn 27.50%,Pi25.00%.In the CP group,the rates of periodontal bacteria were Pg 73.33%,Tf 91.11%,Fn 86.67%,Pi 66.67%.Fn rate was significantly different with degree of the severity in IS combined with CP group.Conclusions:The distribution of putative periodontal pathogens Pg,Tf,Fn and Pi is similar in the CP combined with IS group and CP group.Fn rate was significantly increased with degree of the periodontal severity in the IS combined with CP group.
