根除幽门螺杆菌感染治疗中心性浆液性脉络膜视网膜病变临床分析

摘　　要：目的探讨根除幽门螺杆菌（Hp）感染对治疗中心性浆液性脉络膜视网膜病变（CSC）的临床效果。方法前瞻性临床研究。将2011年4月至2014年10月在聊城市第二人民医院确诊为CSC同时合并Hp感染的51例患者分为A、B两组。A组患者初诊时即接受了根除Hp的药物治疗，B组患者未接受任何治疗。两组患者均接受视力（包括最佳矫正视力）、标准Amsler方格表、色觉检查图、光学相干断层扫描（OCT）等检测。随访18个月。结果A组患者视力在随访的第6、12及18个月均比B组显著提高，差异有统计学意义（P 〈0.05）。两组患者的视物变形率、变暗率及变色率在初诊及随访的12个月内相比差异均无统计学意义（P〉 0.05），而18个月后，A组患者视物变形率、变暗率均比B组明显下降，两组对比差异有统计学意义（P 〈0.05），两组患者视物变色率差异仍无统计学意义（P〉 0.05）。A组患者的视网膜下积液在6个月内既已全部吸收，且A组患者在随访的第3个月及18个月视网膜下积液完全吸收率显著高于B组，差异具有统计学意义（P 〈0.05）。A组患者的复发率低于B组，差异有统计学意义（P 〈0.05）。结论根除Hp感染可提高CSC的治疗效果，降低复发率。Objective To investigate the efficiency of Helicobacter pylori （Hp） infection eradication in treatment of patients with central serous chorioretinopathy （CSC）. Methods A prospective clinical study. Fifty-one patients with CSC who were infected with Hp were divided into group A and group B. The patients of group A received an eradication Hp treatment on the first visit while the patients of group B did not receive any treatment. Visual acuity, color vision plate, Amsler grid and optical coherence tomography （OCT） were examined. The follow-up period was 18 months. Results The visual acuity of the patients in group A increased more than group B after 6 months, which was statistically differences （P 〈0.05）. The frequency of metamorphopsia, dimmed vision between two groups represented statistically differences （P 〈0.05） after 18 months, while color vision did not represent statistically differences （P 〉0.05） during the follow-up period. The disappearance rate of subretinal fluid of the patients in group A was higher than group B in 3 months and 18 months which demonstrated statistical differences （P 〈0.05）. Conclusions Hp infection eradication is effec- tive in the treatment of patients with CSC.
