摘 要:目的分析原发性胰腺淋巴瘤的CT特征及增强特点,以提高对原发性胰腺淋巴瘤的认识。方法回顾性分析2009年7月至2015年11月于我院接受病理检查并确诊的7例原发性胰腺淋巴瘤患者的CT图像特点,并分析其相关病理检查结果。结果 7例患者均为原发性胰腺淋巴瘤,肿块体积较大,平均6.3 cm,与正常胰腺组织分界不清,CT扫描呈稍低密度影,病灶直径均大于5 cm,最大直径为8.3 cm,未出现胰管扩张,动、静脉期轻度强化、渐进性增强,出现类似"血管漂浮征"。结论原发性胰腺淋巴瘤的CT特征显示为病灶较大,密度均匀,边界与正常组织分界不清,未出现胰管扩张,动、静脉期轻度强化、渐进性增强,出现类似"血管漂浮征"特征,结合临床、实验室检查则可明确诊断。Objective To analyze the CT features and enhancement features of primary pancreatic lymphoma in order to improve the un-derstanding of primary pancreatic lymphoma. Methods A retrospective analysis was made on the characteristics of CT images of 7 patients with primary pancreatic lymphoma who were confirmed by pathology examination from July 2009 to November 2015. And the relative results were reviewed. Results All the 7 patients were confirmed as primary pancreatic lymphoma. The tumors were larger in size ( averagely 6.3 cm), and there was no clear demarcation between the normal pancreatic tissues and the tumors. CT scan showed slightly low density, and the diame- ters of the lesions were greater than 5 cm, of which the maximum diameters ranged from 5.1 cm to 8.3 cm. There was no dilated pancreatic duct. In the meantime, the arterial and venous phase showed progressive enhancement and‘blood vessel floating' sign. Conclusion The CT features of primary pancreatic lymphoma showed larger lesions, uniform density, unclear boundary demarcation from the normal tissue, no dilated pancreatic duct, progressive enhancement of arterial and venous phase, similar sign of ‘blood vessel floating'. Combined with clinical and laboratory examination can help to make a definite diagnosis.
