温阳益髓方对于肺癌化疗患者骨髓抑制的临床观察和实验研究

摘　　要：目的:本课题基于"阴阳互根"理论,运用温阳益髓方治疗肺癌化疗后出现的骨髓抑制不良反应,通过临床观察探索中医"阴阳互根"理论在肺癌化疗辅助治疗中的科学内涵,从而丰富中医肿瘤温阳治法,提高疗效。方法:采用临床观察:收集50例证属肺脾阳(气)虚型、气阴两虚型的晚期非小细胞肺癌化疗患者,根据随机原则,将病例平均分成治疗组和对照组。治疗组为在化疗前3 d以及化疗药物使用结束后4 d服用温阳益髓方的非小细胞肺癌患者,共服用7 d。对照组为未服中药制剂的非小细胞肺癌化疗患者。两组均在化疗前第3天查血常规,化疗后的第1、3、5天复查血常规,在治疗组服用温阳益髓方的第1天和服用结束的次日查免疫指标、肿瘤相关抗原、KPS评分以及临床症状,对照组和治疗组在同一天检查,观察骨髓抑制率和程度。结果:血象指标的变化:治疗组的白细胞与对照组相比上升,且有统计意义(P<0.05)。治疗组治疗后患者的神疲乏力、少言懒语、面色白、食欲减退与治疗前以及与对照组化疗后均有统计学意义(P<0.05)。治疗组与对照组化疗后相比较KPS有差异(P<0.05)。肿瘤相关抗原两者比较:治疗组与对照组比较无显著性差异(P>0.05)。虽比较无统计学意义,但是治疗组化疗前后,肿瘤相关抗原呈下降趋势;治疗组下降趋势也较对照组大(P<0.01)。结论:中医扶正治疗均对化疗有一定的骨髓保护和提高外周血细胞的作用,本研究显示:温阳治疗也具有较好的骨髓保护作用,研究结果从一个方面证实了中医"阴阳互根"理论的科学内涵。Objective：The research purpose and significance of this topic：this subject of ＂Yin and Yang mutual root＂ based on the theory of ZHANG Jingyue,using Wenyang Yisui Decoction prevention of dry treatment after chemotherapy of lung cancer of the bone marrow inhibit adverse reactions,through clinical observation and experimental study to explore TCM＂Yang for Yin＂theory in lung cancer adjuvant chemotherapy treatment in the treatment of scientific connotation,so as to enrich the tumour of doctor of traditional Chinese medicine warming Yang treatment and improve the efficacy. Methods：The clinical observation and experimental study of two kinds of methods,clinical observation collect 50 cases of disease belongs to deficiency of lung and spleen Yang（Qi）,Qi and Yin deficiency type of chemotherapy in advanced lung cancer patients,according to the principle of random,patients were divided into treatment group and control group,the chemotherapy were according to the standard of chemotherapy. In the treatment group before chemotherapyfor 3 days to start taking Wenyang Yisui Decoction in patients with lung cancer,taking a total of 7 days. The control group did not take lung cancer chemotherapy patients with traditional Chinese medicine preparation. Chemotherapy for 3 days before the blood tests and chemotherapy after the 1,3,5 day review of blood,in the taking of Wenyang Yisui Decoction first day and taking over the next day check immune index,tumor associated antigen,KPS score and clinical symptoms,control group and treatment group on the same day inspection were observed in bone marrow inhibition rate and extent. Results：Changes of hemogram index：treatment group white cells compared with the control group compared with statistical significance（P〈0.05）,and neutrophil cell,hemoglobin,red blood cell,platelet and the control group compared had no statistical significance（P〈0.05）.
