基于血APP调心方治疗轻度老年痴呆症临床研究

摘　　要：目的:基于外周血APP m RNA、血小板APP异构体比值检测,观察调心方治疗AD的疗效及其机制。方法:以随机数字法将59例病人分入中药组、西药组。中药调心方组为31例,西药多奈哌齐组28例。中药组采用调心方颗粒剂治疗,西药组采用盐酸多奈哌齐片,治疗疗程均为3个月。分别检测两组病人治疗前与治疗结束后3 d内外周血APP m RNA测定(Realtime RT-PCR测定)和血小板APP异构体比率检测(采用免疫沉淀SDS-PAGE及Western Blot电泳法测定)。结果:表明老年痴呆症病人外周血APP m RNA水平在治疗后均有所下降,但差异并无统计学意义,两组间对比亦无统计学差异;反之两组病人外周血APP异构体比值(APP130KD/APP106KD)治疗后则均有上升,治疗前后进行对比差异具有统计学意义,调心方上升水平更大超过多奈哌齐组,与其差异具有统计学意义(P<0.05)。结论:试验表明两药对老年痴呆症均具治疗作用,中药调心方颗粒在改善老年痴呆症方面或更具某种优势。其机制或与其调节老年痴呆症病人体内不同APP的异构体比例有一定关系。Objective：Detection of APP m RNA and platelet APP isomer ratio based on peripheral blood,curative effect and mechanism of treating AD by regulating heart. Methods：59 cases were divided into the traditional Chinese medicine group and the Western medicine group by the random number method. Traditional Chinese medicine decoction group of 31 cases,in the Western medicine group of 28 cases. The Chinese medicine group of Heart Benefiting Recipe Granule treatment,Western medicine group was treated with donepezil hydrochloride tablets,the treatment course was 3 months. Detected blood APP m RNA test and platelet APP isomer ratio before and after treatment for 3 days. Results：APP m RNA levels in peripheral blood of patients with senile dementia were decreased after treatment,but there was no significant difference between the two groups,there was no statistical difference between the two groups,there was no statistical difference between the two groups. Conclusion：Two drugs in the treatment of Alzheimer＇s disease,traditional Chinese medicine prescription has a certain advantage in improving senile dementia. The mechanism may be related to regulating the proportion of different APP isoforms in patients with Alzheimer＇s disease.
