甘遂联合生长抑素治疗重症急性胰腺炎的临床观察

摘　　要：目的:研究甘遂治疗重症急性胰腺炎的临床疗效。方法:将80例重症急性胰腺炎患者按照随机数字表法分为治疗组和对照组各40例,对照组采用予以禁食,持续胃肠减压、营养支持、补液、纠正水、电解质紊乱、酸碱平衡紊乱,抑制胃酸及胰酶分泌等常规治疗,治疗组在对照组的基础上加用甘遂攻里泻下。结果:治疗组40例患者中痊愈18例、好转17例、无效5例,总有效率87.5%,对照组40例患者中痊愈11例、好转16例、无效13例,总有效率67.5%,两组比较有显著性差异(P<0.05),两组主要观察指标恢复时间显示,治疗组腹胀、腹痛和上腹部压痛等症状的恢复时间明显少于对照组,二者有显著性差异(P<0.05)。结论:在西医常规治疗基础上加用中药甘遂,充分发挥中西医结合治疗急性胰腺炎的优势,可提高临床疗效、缩短疗程,值得在临床上进一步推广。Objective： To study the clinical effect of Euphorbia kansui in treating severe acute pancreatitis. Method： 80 cases of severe acute pancreatitis were divided into the treatment group and the control group according to random number table method, with 40 cases in each group, the control group received the routine treatment of fasting, continuing gastrointestinal decompression, nutritional support, fluid infusion, correcting water, electrolyte imbalance, disturbance of acid-base balance, inhibition of gastric acid secretion and trypsin, etc., the treatment group received Euphorbia kansui for endopurgation and purgation. Result： In the treatment group, 18 cases were healed, 17 cases was improved, 5 cases was invalid out of 40 cases of patients, the total effective rate was 87.5%; In the control group, 11 cases were healed, 16 cases was improved, 13 cases was invalid out of 40 cases of patients, the total effective rate was 67.5%, there was significant difference between the two groups （P〈0.05）, the recovery time of abdominal distention, abdominal pain and epigastric tenderness etc. in treatment group was significantly shorter than that of the control group, the difference was of significance（P〈0.05）. Conclusion： On the basis of the routine treatment of western medicine, Euphorbia kansui could improve the clinical curative effect and shorten the treatment course, which brought the full play of the advantage of combination of traditional Chinese and western medicine in the treatment of acute pancreatitis, therefore, it was worthy of further popularizing in the clinic.
