不同剂型曲安奈德治疗糜烂型口腔扁平苔藓的短期疗效评价

摘　　要：目的:观察不同剂型曲安奈德治疗双颊糜烂型口腔扁平苔藓(OLP)的疗效。方法:将120例符合纳入标准的OLP患者随机分为4组(n=30):分别采用局部涂布曲安奈德口腔软膏(A组)、曲安奈德注射液局部封闭(B组)、曲安奈德漱口液(C组)进行治疗,3个实验组及对照组患者均用康复新液含漱,比较4组的糜烂症状及疼痛改善情况(VAS评分)。结果:3个实验组治疗1、2、4周的VAS评分均明显低于对照组,总有效率明显均高于对照组(P<0.05);A组及B组VAS评分和总有效率明显优于C组,差异有统计学意义(P<0.05),但A组与B组间比较差异无统计学意义(P>0.05);A组及B组治疗1、2、4周后病损糜烂面积均值明显均低于对照组(P<0.05),C组治疗1、2周后与对照组无明显差异,治疗4周后明显低于对照组(P<0.05)。结论:曲安奈德治疗OLP疗效较好,口腔软膏是治疗OLP安全有效并且方便的药物。Objective： To observe the efficacy of different forms of triamcinolone acetonide （TA） in the treatment of buecal erosive oral lichen planus （EOLP）. Methods： 120 EOLP patients were randomly divided into 4 groups（ n = 30） and treated with Kangfuxin mouth- wash followed by topical ointment of（TA）, TA injection, TA mouth-wash and only Kangfuxin mouth-wash only, respectively. Before treatment, 1,2 and 4 weeks after treatment, the pain （ VAS score） and erosion area were measured, the effects were compared among groups. Results： The VAS scores of A, B and C groups were significantly lower than those of the control group after 1,2 and 4 week treatment, and the total effective rate was significantly higher than that of the control group, respectively （P 〈 0.05 ）. The VAS score was lower and total effective rate was higher in group A and group B than those in group C （P 〈 0, 05 ）, but there was no statistically sig- nificant difference between group A and B（P 〉 0.05 ）. The average erosion area of group A and B was significantly smaller than that of the control group after l, 2 and 4 week treatment（ P 〈 0, 05 ）. There was no significant difference between group C and the control group after 1 and 2 week treatment（ P 〉 0.05）, but 4 weeks after treatment, the average area of erosion of group C was significantly smaller than that of the control group（ P 〈 0.05）. Conclusion：Triamcinolone acetonide is effective in the treatment of OLP and the oral oint- ment form is safe and convenient for OLP treatment.
