摘 要:目的:观察益气养血泻浊法中药肾衰养真胶囊对慢性肾衰竭(CRF)营养不良大鼠蛋白激酶B(Akt)/叉头转化因子1(Fox O1)信号通路的影响。方法:采用5/6肾切除同时予4%酪蛋白饮食制作CRF营养不良大鼠模型。大鼠随机分为正常组、模型组、中药组和西药组。治疗4周后,检测肌酐(Scr)、尿素氮(BUN)、白蛋白(Alb)、血红蛋白(Hb);蛋白质印迹法(Western Blot)检测骨骼肌磷酸化Akt(p-Akt)蛋白水平;逆转录-实时定量聚合酶链式反应(Real-time PCR)检测骨骼肌肌肉萎缩基因(MAFbx)和肌环指蛋白-1基因(Mu RF-1)和Fox O1 m RNA水平。结果:与模型组比较,中药组大鼠骨骼肌Fox O1 m RNA、MAFbx m RNA和Mu RF-1 m RNA表达水平均显著下降(P<0.01),而p-Akt蛋白表达水平显著上升(P<0.05),同时,大鼠Scr、Alb、Hb均显著上升(P<0.05,P<0.01)。且与西药组比较,中药组Hb显著升高(P<0.01)。结论:益气养血泻浊法中药肾衰养真胶囊能显著改善CRF营养不良大鼠的营养状况,其机制可能通过下调骨骼肌Fox O1 m RNA、MAFbx m RNA和Mu RF-1m RNA表达,上调p-Akt蛋白表达水平,从而减少蛋白分解,改善CRF营养不良。Objective: To investigate the effect of Shenshuai Yangzhen Capsule(SSYZC) on protein kinase B(Akt)/Forkhead box O1(Fox O1) pathway in chronic renal failure(CRF) rats with malnutrition(MN). Methods: SPF grade male SD rats were suffered a 5/6 nephrectomy and 4% casein diet to establish the CRF-MN rat model. Rats were randomly divided into four groups: control group, model group, Chinese medicine group, western medicine group. After being treated for 4 weeks, the renal function related indexes, including serum creatinine(Scr), urea nitrogen(BUN), albumin(Alb), haemoglobin(Hb) were measured. The expression of phospho-Akt(p-Akt) protein in skeletal muscle was measured by Western Blot. The expression of Fox O1, muscle atrophy F box(MAFbx) and Muscle RING-finger protein 1(Mu RF-1) in skeletal muscle was determined by Real-time PCR. Results: Compared with model group, the gene expressions of Fox Ol, MAFbx and Mu RF-1 in skeletal muscle of rats with MN in Chinese medicine group were significantly down-regulated(P〈0.01), but the expression of p-Akt protein was markedly upregulated(P〈0.05). Meanwhile, the levels of Alb and Hb in Chinese medicine group were significantly increased when compared to the model group. Conclusion: SSYZC can improve the malnutrition condition of CRF-MN rat model, which is possibly related to the downregulation of the genes expression of Fox O1 m RNA, MAFbx m RNA and Mu RF-1 m RNA and the upregulation of expression of p-Akt protein in skeletal muscle, resulting in a decrease in the albuminolysis.
