三维立体定位CT引导下^(125)I粒子植入治疗原发性肝癌的疗效及安全性

摘　　要：目的分析三维立体定位下^(125)I粒子植入治疗原发性肝癌的可行性及安全性。方法收集2012年1月至2014年6月唐山人民院收治原发性肝癌患者56例,随机将其分为A组和B组,所有患者均先进行肝动脉化疗栓塞TACE治疗,于治疗后3~14d内,A组28例患者在三维立体定位CT引导下进行^(125)I粒子植入治疗,B组28例患者仅在CT引导下进行^(125)I粒子植入治疗,治疗后随访2~18个月,对两组患者的疗效及安全性进行分析评价。结果治疗后2~18个月随访显示,A组31个病灶完全缓解(CR)8个,部分缓解(PR)16个,稳定(SD)6个,进展(PD)1个,有效率为77.4%。B组32个病灶完全缓解(CR)4个,部分缓解(PR)10个,稳定(SD)8个,进展(PD)10个有效率为43.7%。结论在TACE治疗的基础上,应用三维立体定位技术在CT引导下^(125)I粒子植入比单独CT下引导^(125)I粒子植入治疗原发性肝癌能够取得更好的临床疗效,并提高了治疗的精准性及术后生存率。Objective To analyze the feasibility and the safety under three-dimensional localization of-（125）I grnule implantation in the treatment of primary liver cancer. Methods We collected 56 liver cancer patients who were admitted in Tangshan people hospital from January 2012 to June 2014. Patients were divided into group A and group B. All patients first had the hepatic artery chemotherapy embolism TACE treatment.Three to 14 days after the treatment,group A of 28 patients received-（125）I granule implant the treatment of three-dimensional localization under the CT. Group B of 28 patients had-（125）I granule under the CT guidance.After the treatment,patients were followed-up for 2-18 months. Results A Group A which had 31 focuses showed 8 focuses with complete response（ CR）,16 partial response（ PR）,6 stable disease（ SD）,1progressive disease（ PD）,effectiveness was 77. 4%. Group B with 32 focuses showed 4 CR,th10 PR,8SD,10 PD,effectiveness was 43. 7%. Conclusion On the base of TACE treatment,the application of threedimensional localization technology for-（125）I granule implants under the CT guidance in the treatment of primary liver cancer to be able to obtain appeared to have a better clinical curative effect,enhanced precision treatment and improved survival rate when compared to CT alone guided-（125）I seed implant.
