术前新辅助化疗对局部进展期贲门癌治疗效果的影响研究

摘　　要：目的探讨新辅助化疗在手术治疗局部进展期贲门癌中的意义。方法回顾性分析172例行手术治疗的局部进展期贲门癌患者资料,其中90例行新辅助化疗+手术治疗作为CT-S组,82例行常规手术治疗作为S组,评价新辅助化疗疗效,比较2组围手术期并发症发生率、手术根治性切除率、淋巴结转移率及术后生存率。结果 CT-S组的化疗总有效率为60%(54/90);2组术后并发症发生率比较差异无统计学意义(P>0.05);CT-S组肿瘤根治性切除率明显高于S组(P<0.05),淋巴结转移率明显低于S组(P<0.05),1,3,5年生存率均明显高于S组(P均<0.05)。结论局部进展期贲门癌患者术前行新辅助化疗能提高患者的根治切除率及生存率,降低淋巴结转移率。Objective It is to approach the significance of neoadjuvant chemotherapy before operation in the treatment of local advanced gastric cardial carcinoma. Methods 172 patients of local advanced gastric cardial carcinoma were studied in retrospect. Among them,90 cases treated with neoadjuvant chemotherapy combined with surgery were included in the CT- S group and 82 cases treated with routine surgical treatment were included in the S group,the effect of neoadjuvant chemotherapy were evaluated,and the incidence rate of perioperative complications,radical resection rate,lymph node metastasis rate and postoperative survival rate were observed and compared between the two groups. Results In the CT- S group,the objective remission rate was 60%（ 54 /90）. There was no significant difference in the complication rate after surgery between the two groups（ P 〉0. 05）. In the CT- S group,the radical resection rate of tumor were significantly higher than that in the S group（ P 〈0. 05）,the lymph node metastasis rate was significantly lower than that in the S group（ P 〈0. 05）,and the 1-,3-,5- year survival rate were higher than that in the S group（ all P 〈0. 05）. Conclusion The treatment of neoadjuvant chemotherapy before surgery is available for local advanced gastric cardial carcinoma which can increase the radical resection rate and survival rate as well as decrease the lymph node metastasis rate.
