自研颈椎牵旋试验在颈性眩晕中的诊断价值

摘　　要：目的探讨自研"牵旋试验"在颈性眩晕中的诊断价值。方法收集2012年2月-2014年5月在河北医科大学第二医院康复科门诊就诊并诊断为颈性眩晕的患者150例为研究组,另选取非颈性眩晕患者150例为对照组。采用眩晕障碍量表(DHI)对两组患者进行评分,根据牵旋试验检查前后所得评分(仅包括躯体及功能项)改善情况判定诊断结果,并与常规诊断标准进行Pearson相关性分析,同时记录牵旋试验灵敏度、特异度、似然比(LR)、约登指数(Youden’index)。结果 1颈椎牵旋试验与常规诊断标准呈正相关(r=0.64,P<0.05);2颈椎牵旋试验灵敏度为96%,特异度为94%,阳性似然比为16,阴性LR为0.04,约登指数为0.90。结论 1自研颈椎"牵旋试验"在颈性眩晕的诊断中具有较高的灵敏度及特异度,具有较高的临床诊断价值;2自研颈椎"牵旋试验"作为颈性眩晕的一种诊断及鉴别诊断试验,适于在临床推广应用,具有较好的应用前景。Objective To explore the diagnostic value of ＂cervical pull spin test＂in cervical vertigo. Method 150 patients who had been diagnosed with cervical vertigo were collected in the rehabilitate clinic of Second Hospital of Hebei Medical University from February 2012 to May 2014, as the experimental group. 150 patients who had no cervical vertigo were also collected as the control group. The patients in the two groups were scored by the scale of dizziness handicap inventory DHI before and after the test,according to the score improvement to make a diagnosis,and compared with conventional diagnostic criteria by Pearson correlation analysis.At the same time the results of the experiment were recored： cervical vertigo true positive and false positive, true negative and false negative cases, calculate sensitivity, specificity, likelihood ratio（LR） and Youden Index. Results 1 ＂Cervical pull spin test＂ and conventional diagnostic criteria were positively correlated（r=0.64,P〈0.05）. 2 The sensitivity was 96%, specificity was 94%, positive likelihood ratio was 16, negative likelihood ratio was 0.04, The Youden Index was 0.90. Conclusion 1 ＂Cervical pull spin test＂ has higher sensitivity and specificity in the diagnosis of cervical vertigo, it has high clinical diagnostic value; 2＂Cervical pull spin test ＂is suitable for application in clinical diagnosis and differential diagnosis for cervical vertigo, which has a good application prospect.
