摘　　要：目的了解肺毛霉病患者治疗用药的有效性、安全性和药学监护情况,探讨临床药师如何在临床实践中发挥作用。方法临床药师参与1例肺毛霉病患者的治疗,从肺毛霉病治疗的药物选择、两性霉素B的剂量、疗程以及不良反应监护和处理等方面进行药学服务。结果两性霉素B治疗肺毛霉菌病有确切疗效。两性霉素B的剂量与疗程需根据患者的耐受性和治疗效果进行个体化制定,本患者最高剂量1 mg/(kg·d),总量2.72 g。用药过程中严密监测肾毒性等不良反应并进行相应处理。患者治愈出院,不良反应轻微。结论临床药师对肺毛霉病患者进行药学监护,协助医师制定安全、有效的治疗方案,及时解决药品不良反应,促进临床合理用药。Objective To investigate the safety and efficacy of drug use and pharmaceutical care for one patient with pulmonary mucormycosis, and discuss the role of clinical pharmacists. Methods Clinical pharmacists participated in the treatment for one case of pulmonary mucormycosis, and provided pharmaceutical care such as drug choice, dosage, treatment course, drug adverse reactions（ADR）, etc. Results Amphotericin B was effective for pulmonary mucormycosis. The dosage and course of treatment with amphotericin B should be based on individual tolerance. The highest dose for this patient was 1 mg/（kg·d）, and the cumulative dose was 2.72 g. Pharmacists dealt with the ADR such as renal toxicity timely. The patient was cured with mild adverse reactions. Conclusion Clinical pharmacists have provided pharmaceutical care for the one patient with pulmonary mucormycosis, developed safe and effective treatment plan, and dealt with ADR timely.
