不同剂量加巴喷丁治疗尿毒症顽固性瘙痒的临床观察

摘　　要：目的:探讨加巴喷丁不同剂量服药情况下治疗尿毒症顽固性瘙痒的效果。方法:将90例尿毒症顽固性瘙痒患者随机分为低、中、高剂量三组,三组患者均采取透析加加巴喷丁治疗,其中低、中、高剂量组剂量依次为100mg、200mg、300mg加巴喷丁胶囊,观察记录三组患者临床效果、不良反应,治疗前后检查肝功能与电解质,以及治疗前后疼痛VAS评分与瘙痒VAG评分,并对比分析。结果:中剂量组疗效最好,明显优于低剂量组(P<0.05),但与高剂量组无差异(P>0.05);低剂量组不良反应最少,与高剂量组比较差异明显(P<0.05),与中剂量组比较无明显差异(P>0.05);三组患者治疗前VAS评分与VAG评分无明显差异(P>0.05),治疗后均有明显下降(P<0.05),中剂量组治疗后明显最低(P<0.05)。结论:尿毒症顽固性瘙痒患者采取200mg加巴喷丁治疗效果最好,安全性高,可更好地改善VAS评分与VAG评分,值得借鉴。Objective：To investigate the effect of different doses of gabapentin in the treatment of refractory pruritus in uremic patients.Methods：90 cases of uremic patients with refractory itching were randomly divided into three groups：low dose group,high dose group,low dose group,high dose group and high dose group.The VAS and VAG scores of pain and VAS before and after treatment were compared and analyzed,and the liver function and electrolytes were observed before and after treatment.Results：The middle dose group had the best effect,which was better than the low dose group（P〈0.05）,but there was no significant difference between the high dose group and the high dose group（P〉0.05）;（P〈0.05）,and there was no significant difference between the two groups（P〉0.05）.There was no significant difference between VAS and VAG before treatment（P〉0.05）Dose group after treatment was significantly lower（P〈0.05）.Conclusion：200mg of gabapentin is the best and safer for uremic patients with refractory pruritus,so it is better to improve the score of VAS and VAG.
