腹腔镜辅助与开腹结直肠癌根治术的临床疗效比较

摘　　要：目的比较腹腔镜辅助与开腹结直肠癌根治术治疗结直肠癌的临床疗效,并根据结果进一步分析腹腔镜辅助结直肠癌根治术存在的问题以及特有的优势。方法 120例结直肠癌患者,将其随机分为观察组和对照组,各60例。观察组患者进行腹腔镜辅助结直肠癌根治术进行治疗,对照组患者进行开腹结直肠癌根治术进行治疗。观察两组患者的相关临床指标以及并发症、远处转移情况。结果观察组术中出血量、术后排气时间、术后排便时间、开始进流食时间、住院时间、切口长度均优于对照组,差异具有统计学意义(P<0.05)。观察组并发症发生率为5.0%,低于对照组的30.0%,差异具有统计学意义(P<0.05);观察组远处转移率为1.7%,低于对照组的11.7%,差异具有统计学意义(P<0.05)。结论腹腔镜辅助结直肠癌根治术治疗结直肠癌的效果明显,安全有效,可以减轻患者的痛苦,在临床上是值得推广使用的。Objective To compare clinical effect by laparoscope-assisted and laparotomy radical operation of colorectal cancer in the treatment of colorectal cancer, and to further analyze issues and advantages of laparoscope-assisted radical operation of colorectal cancer. Methods A total of 120 patients with colorectal cancer were randomly divided into observation group and control group, with 60 cases in each group. The observation group received laparoscope-assisted radical operation of colorectal cancer, and the control group received laparotomy radical operation of colorectal cancer. Observation was made on related clinical indexes, complications and distant metastasis in both groups. Results The observation group had all better intraoperative bleeding volume, postoperative exhaust time, postoperative defecation time, liquid diet start time, hospital stay time, and incision length than the control group. Their difference had statistical significance（P〈0.05）. The observation group had lower incidence of complications as 5.0% than 30.0% in the control group, and the difference had statistical significance（P〈0.05）. The observation group also had lower distant metastasis rate as 1.7% than 11.7% in the control group, and the difference had statistical significance（P〈0.05）. Conclusion Laparoscopeassisted radical operation of colorectal cancer shows remarkable effect in treating colorectal cancer. This method is safe and effective, and it can relieve pain in patients. It is worth clinical promotion and application.
