摘 要:目的 制定简便易行的HAVE评分对非溶栓性脑梗死出血性转化(HT)预测研究。方法 回顾性分析深圳市人民医院2008年~2010年间发病48 h内NIHSS评分≥5分的95例急性脑梗死患者,通过头部CT或MRI监测2 w内非溶栓治疗HT的发生,对HT危险因素进行综合分析,并对危险因素进行量化评分。结果 模型(HAVE)中共纳入4个危险因素:3级高血压(Hypertension)、心房颤动(Atrial fibrillation)、梗死体积(Volume)及高龄(Elderly),总分共8分,其中评分大于等于4分即为出血性转化极高危人群,该评分对发生脑梗死出血性转化预测的灵敏度为0.73,特异度为0.84。结论 HAVE评分能够快速识别脑梗死出血性转化高危人群,具有良好的敏感性及特异性。Objective To formulate a simple and practical HAVE Model for Prediction on None -thrombolytic Hemorrhage Transformation .Methods Retrospective analysis on 95 patients within 48 hours after attack of acute cerebral infarction whose NIHSS scores were equal or greater than 5 between year of 2008 to 2010 in Shenzhen people ’ s hospital . Mornitoring the hemorrhage transformation with non thrombolytic treatment in 2 weeks through brain CT or MRI .Compre-hensive analysis on hemorrhage transformation risk factors ,and quantitative rating on them .Results HAVE Model includ-ed 4 risk factors,which were Hypertension of level three ,Atrial fibrillation,Volume of infraction and Elderly .The total score was 8.The patients with score equal or greater than 4 were predicted to be extreme high risk of hemorrhage transformation . The sensitivity of this prediction model was 0.73,while the specificity was 0.84.Conclusion HAVE score can rapid iden-tify the high risk patients of hemorrhage transformation with favorable sensitivity and specificity.
