摘 要:目的 探讨肺腺癌组织中 ALK 和 ROS1 融合基因发生率及其与临床病理学指标的关系。方法 应用荧光原位杂交 ( FISH )技术分别检测 332 例肺腺癌组织中 ALK 和 ROS1 融合基因表达情况,同时分析患者的性别、年龄、吸烟史、临床分期及组织学类型等临床病理特征,并探讨它们之间的相关性。结果 332 例肺腺癌组织中, 28 例 ( 8.4% )存在 ALK 融合基因, ALK 融合基因阳性在性别、年龄、吸烟史、临床分期方 面 差异 均有 统计 学意 义; 13 例 ( 3.9% )有 ROS1 融 合基 因,ROS1 融合基因阳性在性别、年龄、吸烟史及临床分期方面差异均无统计学意义;未发现 ALK 与ROS1 基因同时突变的患者。结论 ALK 和 ROS1 融合基因在肺腺癌组织中突变率较低,代表了特定的分子亚型,多见于年轻、非吸烟、女性、临床分期较晚的患者,检测 ALK 及 ROS1 基因融合,对临床靶向治疗有重要意义。Objective To explore the frequency of ALK and ROS1 fusion genes in lung adenocarcinomas and its Correlation with clinicopathologic features. Methods ALK and ROS1 fusion genes from samples of 332 patients with lung adenocarcinomas were detected by fluorescence in situ hybridization. The relationship between ALK, ROS1 fusion genes and clinicopathologic features was analyzed. Results ALK fusion gene was detected in 28 of 332 lung adenocarcinomas samples (8.4%). There was statistical difference in the gender, age, smoking history and clinical stage of patients. 13 patients with ROS1 fusion gene were identified among the 332 lung adenocarcinomas samples, which was not correlated with the gender, age, smoking history and clinical stage of patients. None of the cases had concurrent ALK and ROS1 fusion. Conclusions The gene fusion rates of ALK and ROS1 are low in lung adenocarcinomas, and may be defined as special molecular subtype. ALK and ROS1 fusion genes occur more frequently in younger and non smoking female patients with advanced stage. Genetic testing has important significance to guide clinical targeted therapy.
