多西紫杉醇联合同步放疗治疗局部晚期非小细胞肺癌的临床观察

摘　　要：目的 通过对比三维适形放疗联合化疗同步治疗局部晚期非小细胞肺癌的近期临床效果,评价其临床应用价值和意义。方法 将我院 2013 年 12 月至 2015 年 12 月期间收治的 60 例局部晚期非小细胞肺癌患者作为本次的研究对象,根据随机数字方法分为对照组和实验组,每组 30 例。对照组患者给予放化疗序贯疗法,实验组患者给予三维适形放疗同步多西紫杉醇化疗。对比分析治疗后 2组患者的近期临床疗效、不良反应发生率、 1 年和 2 年的生存率及死亡率,分析三维适形放疗联合化疗同步治疗局部晚期非小细胞肺癌的临床应用价值和意义。结果 治疗后实验组患者的近期总有效率（86.7% ）高于对照组 （ 56.7% ）,差异有统计学意义 （ χ^2 =6.893 ,P 〈0.05 ）。治疗后对照组患者的不良反应发生率为 26.7% ,实验组为 33.3% , 2 组比较差异无统计学意义 （χ^2 5.638 ,P 〉0.05 ）。实验组患者 1 年生存率为 66.7% ,死亡率为 33.3% ; 2 年生存率为 53.3% ,死亡率为 46.7% 。对照组患者 1 年生存率为 43.3% ,死亡率为 56.7% ; 2 年生存率为 33.3% ,死亡率为 66.7% 。 2 组患者 1 年、 2年生存率比较差异均有统计学意义 （χ^2 值分别为 4.299 、 4.443 ,P 值均 〈0.05 ）。结论 采用三维适形放疗联合化疗同步治疗局部晚期非小细胞肺癌的近期疗效较好, 1 年、 2 年的生存率高,具有一定的临床应用价值。Objective To evaluate the clinical application value and significance by comparing the clinical effect of three dimensional conformal radiotherapy combined with chemotherapy on locally advanced non-small cell lung cancer. Methods 60 patients with locally advanced non small ceil lung cancer treated in our hospital from December 2013 to December 2015 were selected as the research object. According to the method of random number, the patients were divided into control group and experimental group, 30 cases in each group. The patients in control group were treated with radiotherapy and chemotherapy, and the patients in experimental group were treated with three dimensional conformal radiotherapy combined with docetaxel chemotherapy. After treatment, the clinical efficacy, incidence of adverse reactions, one-year and two-year survival rate and mortality rate of the two groups were compared to analyze the value and significance of three dimensional conformal radiotherapy combined with chemotherapy for locally advanced non-small cell lung cancer. Results After treatment, the total effective rate in the experimental group was higher than that in the control group （86.7% vs 56.7%, x^2=6.893, P 〈0.05）. The incidence of adverse reactions in the control group and experimental group was 26.7% and 33.3%, there was no significant difference in the adverse reaction rates between the two groups （ x^2=5. 638, P 〉0.05）.In the experimental group, one-year survival rate and mortality rate were 66.7% and 33.3%,two year survival rate and mortality rate were 53.3% and 46.7%. In the control group, one year survival rate and mortality rate were 43.3% and 56.7%, two year survival rate and mortality rate were 33.3 % and 66.7 %.
