MTDH和HIF-1α在复发转移性乳腺癌表达及其临床意义的研究

摘　　要：目的检测乳腺癌患者病理组织及外周血中MTDH和HIF-1α的表达情况,以及与乳腺癌临床病理参数的相互关系。方法采用细胞免疫化学、免疫组化、流式细胞术分别对乳腺癌细胞系、病理组织及外周血进行检测。结果 (1)MTDH主要表达于乳腺癌细胞的细胞膜与细胞浆,在组织中少部分表达于细胞核,HIF-1α主要表达于细胞浆与细胞核;(2)在免疫组化检测中,MTDH阳性率为63.3%,HIF-1α阳性率为23.3%;(3)MTDH表达情况与肿瘤的临床分期(P=0.010)、转移部位(P=0.003)、疾病进展时间(TTP)(P=0.002)有相关性,与年龄、原发淋巴结转移、HER-2、ER、PR等无相关性,HIF-1α的表达情况与乳腺癌病理参数均无明确相关性。MTDH与HIF-1α线性回归系数R=0.365,P<0.05有统计学意义。结论在MTDH和HIF-α与乳腺癌复发转移有明确相关性,对乳腺癌的疾病进展有着重要的作用。Objective： The objective is not only to detect the expression of MTDH and HIF-1α with the pathological tissues in metastasis breast cancer but also to study on interaction with MTDH,HIF-1α and clinical information of breast cancer. Methods： Cellular immunochemical,Immunohistochemical assay,flow cytometry（ FCM） are used respectively to detect the expression of MTDH and HIF-1α protein in breast cancer cell line MCF-7,breast cancer cell line MCF-7,MDAMB-231 and the peripheral blood. Results：（ 1） MTDH describes largely the cellular membrane and the cytoplasm of breast cancer cells,while only a few of MTDH of pathological tissues may illustrate the cell nucleus. Meanwhile,HIF-1α is used for explaining the cytoplasm and the cellular nucleus.（ 2） In immunohistochemical study,the positive rates of MTDH in breast cancer patients is 63. 3%,and the positive rates of HIF-1αin breast cancer patients is 23. 3%.（ 3） The correlation is existed between the expression of MTDH in breast cancer with the clinical stage（ P = 0. 010） 、the metastatic sites（ P = 0.003）,time to progression（ P = 0. 002） rather than with patients age,lymph node metastasis,ER,PR and HER-2. The expression of HIF-1α had no explicit correlation with the clinical pathological parameters of the breast cancer patients. There is certian staisstical significance between MTDH with HIF-1α（ R = 0. 365,P〈0. 05）. Conclusions： This paper showed that Significant correlation was found between the MTDH 、HIF-1α for the metastasis and the angiogenesis of breast cancer.MTDH gene and HIF-1α gene may play important roles in breast cancer.
