FAP-α在胃癌组织中的表达及其临床意义

摘　　要：目的:检测成纤维细胞激活蛋白α(FAP-α)在胃癌组织中的表达情况,分析其表达的临床意义。方法:收集自2008年6月1日至2010年12月30日在广东医学院附属医院行手术切除,并且全部经过病理诊断证实为胃腺癌的患者98例,伴癌旁组织者20例。同时追踪随访患者生存情况截止至2015年12月30日。收集经胃镜取材诊断为胃炎的患者10例。应用免疫组织化学方法检测FAP-α在胃癌组织、癌旁组织及胃炎组织中的表达。分析FAP-α的表达与胃癌临床病理特征的关系及患者预后的相关性。结果:FAP-α在癌旁组织及胃炎组织中不表达,在胃癌组织中主要分布于胃癌实质腺细胞,在胃癌间质中呈少量分布。FAP-α在胃癌组织中的表达与性别、年龄、肿瘤分级、肿瘤大小、浸润深度及淋巴结转移等不具有相关性(P>0.05),与患者术后生存期呈负相关,即FAP-α表达越多,患者术后生存期越短(P=0.035)。结论:FAP-α主要表达于胃癌实质腺细胞。FAP-α的表达与患者性别、年龄、肿瘤分级、肿瘤大小、浸润深度及淋巴结转移等因素之间不具有相关性,与患者术后生存期呈负相关,可用于作为评估患者预后的指标。Objective：To determine the expression of fibroblast activation protein-α（FAP-α） in gastric carcinoma and its clinical significance. Methods：Included in this study were 98 samples of gastric adenocarcinoma and 20 samples of adjacent tissues from patients who received surgical resection,and were pathologically confirmed to have gastric adenocarcinoma in the Affiliated Hospital of Guangdong Medical College between June 1,2008 and December 30,2010. The survival of patients was followed up as of December 30,2015. Ten samples were collected via endoscopy from patients who were diagnosed with gastritis. Immunohistochemistry was used to detect the expression of FAP-α in gastric cancer tissues,adjacent tissues and gastritis tissue. Correlation of FAP-α expression with clinicopathological features of gastric carcinoma and patient prognosis were analyzed. Results：FAP-α was not expressed in cancer adjacent tissues or gastritis. The expression of FAP-α in gastric carcinoma was mainly localized in glandular cells of the tumor parenchyma,compared with its sparse expression in the tumor stroma. FAP-αexpression in gastric carcinoma was not correlated with gender,age,tumor grading,tumor size,depth of invasion and lymph node metastasis （P〉0.05）,but was negatively correlated withnbsp;postoperative survival. In other words,higher expression level of FAP-α was associated with shorter length of postoperative survival （P=0. 035）. Conclusion：FAP-α is mainly expressed in parenchymal glandular cells of gastric carcinoma. FAP-α expression is not correlated with gender,age,tumor grading,tumor size,depth of invasion and lymph node metastasis,and is negatively correlated with the survival time of patients. Therefore, FAP-α can be as a biomarker for patient prognosis.
