非动脉炎性前部缺血性视神经病变与颈内动脉弹性功能参数相关性研究

摘　　要：目的探讨非动脉炎性前部缺血性视神经病变与颈内动脉狭窄及颈内动脉弹性功能参数的相关性。方法选择2014年1月～2015年1月在北京友谊医院眼科确诊为非动脉炎性前部缺血性视神经病变患者57例（57眼）作为研究组，对照组选取无非动脉炎性前部缺血性视神经病变者55例（55眼），均进行多普勒彩超检查，测定颈内动脉的收缩期内径（Ds）、舒张期内径（Dd）、收缩期血流峰值（Vs）、舒张期血流峰值（Vd）、平均血流速度（Vm）、内一中膜厚度（IMT）、斑块形成情况；计算颈内动脉弹性功能参数：管壁运动度（AD）、动脉紧张度、扩张性、压力一应变性系数（Ep）、僵硬度指数（B）、顺应性（AC）、阻力指数（RI）。比较分析两组指标。结果研究组的斑块检出率明显高于对照组（P〈0．05）；研究组颈内动脉狭窄发生率明显高于对照组（P〈0．05）；研究组颈内动脉Vm、RI、动脉紧张度、动脉扩张性、Ep及p与对照组比较，差异有统计学意义（P〈0．05），而颈内动脉的IMT、AD及AC与对照组比较，差异无统计学意义（P〉0．05）。结论非动脉炎性前部缺血性视神经病变的发生与颈内动脉狭窄及颈内动脉弹性功能参数的改变有一定的相关性。Objective To investigate the correlation between non-arteriticanterior ischemic optic neuropathy （NAION） and elasticity parameters of internal carotid artery. Methods 57 patients with NAION who presented to Beijing Friendship Hospital from January 2014 to January 2015 were selected as the NAION group and 55 patients were enrolled as control group. All the participants were examined with color Doppler ultrasonography. The following indexes were mea- sured： systolic inner diameter （Ds）, diastolic diameter （Dd）, peak systolic （Vs）, diastolic peak （Vd）, the average blood flow velocity （Vm）, intima-media thickness （IMT） and plaque formation. The following values were calculated： the internal carotid artery elastic parameters by relevant formula; wall motion degree （AD）, arterial tension, arterial expansion, pressure-strain elastic modulus （Ep）, stiffness index （[3）, arterial compliance （AC） and resistance index （RI）. The elasticity parameters of internal carotid artery and other indexes were compared. Results Detection rate of carotid plaque and rate of internal carotid artery stenosis of NAION group were significantly higher than those of control group （P 〈 0.05）, the Vm, RI, arterial tension, arterial expansion, Ep and β of NAION group had significant differences compared with control group （P 〈 0.05）, IMT,AD and AC of NAION group had no significant differences compared with control group（P 〉 0.05）. Conclusion NAION correlates with rate of internal carotid artery stenosis and elasticity parameters of internal carotid artery.
