摘 要:目的:研究口腔扁平苔藓组织中STATs家族表达与外周血PD-1、PD-L1表达及免疫功能的相关性。方法:选择在我院诊断为口腔扁平苔藓的47例患者作为研究的扁平苔藓组(OLP组),选择同期体检的健康志愿者作为研究的对照组。采集外周血单个核细胞并检测PD-1、PD-L1以及免疫细胞表面标志分子的表达量,采集血清并测定Th1、Th2细胞因子及免疫球蛋白的含量,采集口腔扁平苔藓病灶组织及病灶旁正常组织并测定STATs家族的表达量。结果:病灶组织中p-STAT1、p-STAT3、pSTAT5a的表达量显著高于正常组织,p-STAT2、p-STAT4、p-STAT5b的表达量与正常组织比较无显著性差异;OLP组外周血单个核细胞中PD-1、PD-L1的mRNA表达量以及CD19^+的平均荧光强度显著高于对照组且与p-STAT1、p-STAT3、p-STAT5a的表达量呈正相关,CD3^+、CD4^+、CD8^+、CD16^+CD56^+的平均荧光强度显著低于对照组且与p-STAT1、p-STAT3、p-STAT5a的表达量呈负相关;OLP组血清中IFN-γ、IL-2的含量显著低于对照组且与p-STAT1、p-STAT3、p-STAT5a的表达量呈负相关,IL-4、IL-10、IgG、IgM、IgA的含量显著高于对照组且与p-STAT1、p-STAT3、p-STAT5a的表达量呈正相关。结论:口腔扁平苔藓组织中p-STAT1、p-STAT3、p-STAT5a的表达量异常升高,PD-1/PDL1所介导的Th1/Th2细胞免疫及体液免疫紊乱与病灶中STAT分子表达量的改变密切相关。Objective: To study the correlation of STATs family expression in oral lichen planus tissue with peripheral blood PD-1 and PD-L1 expression as well as immune function. Methods.. A total of 47 patients diagnosed with oral lichen planus in our hospital were enrolled as the oral lichen planus group (OLP group), and healthy volunteers receiving physical examina- tion during the same period were selected as the control group. Peripheral blood mononuclear cells were collected to detect the expression of PD-1, PD-L1 and immune cell surface marker molecules, serum was collected to detect the content of Th1 and Th2 cytokines as well as immunoglobulin, and oral lichen planus lesion tissue and adjacent normal tissue were collected to de- termine STATs family expression. Results: p-STAT1, p-STAT3 and p-STAT5a expression in lesion tissue were significantlyhigher than those in normal tissue while p-STAT2, p-STAT4 and p-STAT5b expression were not significantly different from those in normal tissue; PD-1 and PD-L1 mRNA expression as well as the mean fluorescence intensity of CD19+ in peripheral blood mononuclear ceils of OLP group were significantly higher than those of control group and positively correlated with p- STAT1, p-STAT3 and p-STATSa expression while the mean fluorescence intensity of CD3+ , CD4+ , CD8+ and CD16+ CD56+ were significantly lower than those of control group and negatively correlated with p-STAT1, p-STAT3 and p-STAT5a expres- sion; serum IFN-γ and IL-2 content of OLP group were significantly lower than those of control group and negatively correla- ted with p-STAT1, p-STAT3 and p-STAT5a expression while IL-4, IL-10, IgG, IgM and IgA content were significantly high- er than those of control group and positively correlated with p-STAT1, p-STAT3 and p-STAT5a expression. Conclusion: p- STAT1, p-STAT3 and p-STAT5a expression abnormally increase in oral lichen planus tissues, and the Th1/Th2 cellular im- munity and humoral immunity disorder mediated by PD-1/PD-L1 is closely related to the
