鲜胚移植和冻融胚胎移植中单卵双胎发生的相关因素分析

摘　　要：目的探讨鲜胚移植和冻融胚胎移植中单卵双胎发生的相关影响因素。方法回顾性分析2014年1月至2015年12月间在我院实施IVF/ICSI-ET并获得临床妊娠的6 893个周期,其中鲜胚移植周期3 355个,冻融胚胎移植周期3 538个。按照患者年龄、移植胚胎种类、授精方式、促排卵方案的不同,以及是否行辅助孵化分为不同组别,比较各组的单卵双胎妊娠发生率,并分析其可能的影响因素。结果6 893个临床妊娠周期中共发生单卵双胎妊娠86例,发生率为1.25%;各年龄段、辅助孵化与否、鲜胚移植周期中不同授精方式及不同促排卵方案的单卵双胎妊娠发生率比较均无显著性差异(P均>0.05);囊胚移植组的单卵双胎妊娠发生率(1.92%)则显著高于第3天卵裂期胚胎移植组(0.65%)(P<0.01)。结论辅助生殖助孕治疗中单卵双胎妊娠的发生可能与囊胚移植显著相关,与年龄、受精方式、辅助孵化与否、促排方案则无显著相关,但仍需更多数据证实。Objective： To investigate the factors related to incidence of monozygotic twin in fresh or frozenthawed embryo transfer.Methods： The incidence of monozygotic twin pregnancy in 6 893IVF/ICSI-ET cycles including 3 355 fresh cycles and 3 538frozen-thawed cycles in the Reproductive Center of Northwest Women and Children Hospital from January 2014 to December 2015 were retrospectively analyzed.The cycles were divided into different groups according to the related factors such as patient＇s ages,Day 3embryos or blastocyst transfer,IVF or ICSI,controlled ovarian hyperstimulation（COH）protocol,and assisted hatching etc.The incidence of monozygotic twin pregnancy was compared among the groups and the possible influencing factors were analyzed.Results： Eighty six cases of monozygotic twin pregnancy occurred in 6 893 clinical pregnancies,and the incidence was 1.25%.The incidence of monozygotic twin pregnancy in the different groups divided by ages,assisted hatching,fertilization method,COH protocol had no significant difference（P〉0.05）.The incidence of monozygotic twin pregnancy in the blastocyst transfer group（1.92% vs.0.65%）was significantly higher than that in Day 3cleavage embryo transfer group（P〈0.01）.Conclusions： The incidence of monozygotic twin pregnancy is significantly correlated with blastocyst or embryos transfer,and there are no significant correlations with age,fertilization method,assisted hatching or COH,but more investigations are needed.
