吉西他滨联合替吉奥化疗在胰腺癌根治术后患者中的应用

摘　　要：目的探讨吉西他滨联合替吉奥化疗在胰腺癌根治术患者术后的疗效。方法选取2012年4月至2014年3月间大连市第三人民医院收治的72例胰腺癌根治术后患者,采用随机数字表法随机分为观察组和对照组,每组36例。观察组患者采用吉西他滨联合替吉奥化疗,对照组患者采用吉西他滨联合奥沙利铂化疗。比较两组患者临床疗效、临床受益率、不良反应、1年内死亡率和治疗前后生活质量的差异。结果两组患者的有效率和疾病控制率方面差异无统计学意义(P>0.05)。观察组患者临床受益率高于对照组,组间差异有统计学意义(P<0.05)。观察组患者不良反应发生率较对照组低,差异有统计学意义(P<0.05)。两组患者1年内死亡率差异无统计学意义(P>0.05)。治疗前,两组患者生活质量评分差异无统计学意义(P>0.05)。治疗后,观察组生活质量得分高于对照组,组间差异有统计学意义(P<0.05)。结论吉西他滨联合替吉奥化疗可有效提高胰腺癌患者根治术后的临床受益,并减少不良反应,改善生活质量。Objective To explore the use of gemcitabine plus tegafur chemotherapy in patients with pancreatic cancer after surgery. Methods From April 2012 to March 2014,72 patients who had undergone radical pancreatic surgery for pancreatic cancer at The Third People＇s Hospital of Dalian were enrolled in the study and randomly divided into an observation group and a control group with 36 patients in each group.Patients in the observation group received gemcitabine plus tegafur chemotherapy and those in the control group received gemcitabine plus oxaliplatin chemotherapy. Clinical efficacy,clinical benefit rate,toxicity,1-year mortality and quality of life before and after treatment were compared between the two groups.Results The efficiency and disease control rate showed no significant difference between the two groups（ P〉0. 05）. The clinical benefit rate was higher in the observation group than the control group（ P〈0. 05）. The incidence of toxicity was lower in the observation group than that of the control group（ P〈0. 05）. One year mortality showed no significant difference（ P〉0. 05）. There was no significant difference in the score of quality of life between the two groups before the treatment（ P〉0. 05）. After the treatment,quality of life score was higher in the observation group（ P〈0. 05）. Conclusion Gemcitabine combined with tegafur chemotherapy can increase clinical benefits,reduce toxicity and improve quality of life in patients who had undergone radical surgery for pancreatic cancer.
