摘  要:目的探讨新生儿缺氧缺血性脑病(HIE)中血清前白蛋白(PA)与新生儿行为神经测定(NBNA)的相关性。方法选取2014年1月至2015年12月新生儿重症监护室足月新生儿24例,其中男11例,女13例。根据2005年修订的HIE诊断标准和临床分度将患儿分为轻度HIE组(n=11)、中度HIE组(n=9)、重度HIE组(n=4)和正常足月新生儿组(正常对照组,n=20);各组新生儿分别于出生后第1、3、10天取股静脉血,检测血清PA水平,并对患儿作相应的NBNA评分。结果出生后第1、3天,血清PA水平重度HIE组<中度HIE组<轻度HIE组<正常对照组(P<0.01);出生后第1、3、10天,NBNA评分重度HIE组<中度HIE组<轻度HIE组<正常对照组(P<0.01);重度HIE组、中度HIE组和轻度HIE组经治疗后,第1天血清PA值和NBNA评分均低于第10天(P<0.01)。重度HIE组、中度HIE组和轻度HIE组第1天血清PA与NBNA评分呈正相关性(r=0.554,P<0.01);第10天血清PA与NBNA评分呈正相关性(r=0.344,P<0.01)。结论 HIE越严重的患儿,血清PA和NBNA评分越低,通过积极对症支持治疗后,血清PA值和NBNA评分均升高,表明血清PA和NBNA评分可作为HIE早期诊断及判断预后的指标。Objective To analyze the association between serum prealbumin( PA) level and neonatal behavioral neurological assessment( NBNA) in neonates with hypoxic-ischemic encephalopathy( HIE). Methods A total of 24 neonates including 11 males and 13 females were selected from HIE newborns admitted to Jinan Central Hospital during Jan. 2014 and Dec. 2015. The neonates were divided into 3 groups according to the HIE diagnostic criteria and dinical grading in 2005,mild( n = 11),moderate( n = 9),and severe( n = 4),and another 20 healthy newborns served as controls. Blood samples and NBNA scores were collected from all newborns on day 1,3,and 10 after birth,to detect the serum PA values. Results The Bonferroni's test indicated that on day 1 and 3,there existed such relationship among PA level of 4 groups: PA( severe) 〈PA( moderate) 〈PA( mild) 〈PA( control),( P〈0. 01),so did NBNA scores on day 1,3 and 10( P〈0. 01). Besides,the Pearson's Correlation Coefficient test showed significant positive linear correlation between PA level and NBNA scores of each HIE group,on day 1( r = 0. 554,P〈0. 01),after treatment,and day 10( r = 0. 344,P〈0. 01). Conclusion As the clinical grading becomes higher,the serumPA values and the NBNA scores are lower. Along with the recovery of disease,the serum PA values and NBNA scores gradually increase. The serum PA combined with NBNA scoring could be used as the indicator of early diagnosis and prognostic evaluation of neonatal HIE.
