摘 要:目的:采用Meta分析方法评价let-7低表达在肺癌预后中的作用及高迁移率旋蛋白A2(HMGA2)表达与肺癌关系的临床意义。方法:计算机检索PubMed、Web of Science、Medline、Cochrane Library、Google、Scholar、CNKI、CBM、万方和维普等数据库,收集关于let-7低表达与肺癌预后及HMGA2与肺癌发生发展的关联性研究,采用StataSE 12.0软件进行Meta分析。结果:共纳入10篇关于let-7低表达与肺癌预后关系的文献用于Meta分析,均为非小细胞肺癌患者。let-7低表达预示着肺癌患者较差的总生存率,风险比(HR)的合并值为1.55(95%CI:1.16~2.09,P〈0.05)。共纳入9篇关于HMGA2表达与非小细胞肺癌发生发展关系的病例对照研究用于Meta分析,肺癌组织中HMGA2表达阳性率明显高于癌旁正常组织(OR=5.92,95%CI:3.96~8.85,P=0.000)。HMGA2在肺癌组织的表达与肺癌TNM分期、有无淋巴结转移有明显关联(OR=0.26,95%CI:0.16~0.41,P〈0.05;OR=0.29,95%CI:0.12~0.71,P〈0.05)。结论:肺癌患者中let-7低表达是肺癌患者预后的危险因素。HMGA2表达阳性率越高,肺癌发生的概率就越高,越具有侵袭性。Objective:To evaluate the role of low expression of let-7in the prognosis of lung cancer and the clinical significance of relationship between the HMGA2 expression and lung cancer by Meta-analysis method.Methods:The studies about the correlation between the low expression of let-7and the prognosis of lung cancer and the correlation between the HMGA2 expression and the occurrence and development of lung cancer were collected by searching PubMed,Web of Science,Medline,Cochrane Library,Google,Scholar,CNKI,CBM,and Wanfang,and VIP databases.The StataSE 12.0software was used for this Meta-analysis.Results:A total of 10 studies about the relationship between the low expression of let-7and the prognosis of lung cancer(the patients with nonsmall cell lung cancer)were enrolled in this Meta-analysis.The low expression of let-7indicated a poor overall survival rate,and the combined value of hazard ratio(HR)was 1.55(95%CI:1.16-2.09,P〈0.05).A total of 9 case-control studies about the relatlonship between the expression of HMGA2 and the occurrence and development of non-small cell lung cancer were selected for this Meta-analysis.The positive expression rate of HMGA2 in lung cancer tissue was significantly higher than that in paracancer normal tissue(OR=5.92,95% CI:3.96-8.85,P=0.000).The expression of HMAG2 in lung cancer tissue was pasitively related with the TNM stage and the lymph node metastasis of the patients(OR=0.26,95%CI:0.16-0.41,P〈0.05;OR=0.29,95%CI:0.12-0.71,P〈0.05).Conclusion:The low expression of let-7is closely related to the poor prognosis of lung cancer patients.The highly positive expression rate of HMGA2 in lung cancer tissue is positively correlated with the occurrence and invasiveness of lung cancer.
