腹腔镜下壶腹部癌合并肝硬化行胰十二指肠切除术

摘　　要：完全腹腔镜下的胰十二指肠切除术目前在国内逐渐开展起来,技术要求较高,尤其对于个别特殊的病例,难度较大。此次视频介绍我们为一例壶腹部肿瘤合并乙型肝炎后肝硬化的患者,顺利进行了腹腔镜下胰十二指肠切除术。术中患者肝门部血管曲张严重,手术创面大,手术组医生操作精细,历时9个小时,仅出血100 ml,术中输注血浆600 ml,术后恢复顺利,随访至今未见并发症。Currently, total laparoscopic pancreatoduodenectomy is gradually carried out in China. However, it is still a kind of laborious surgery to be performed, especially for some special cases which are challenging and difficult for the surgeon. In this video, we introduce a periainpullary tumor patient combing with posthepatitic cirrhosis for who were treated with laparoscopic pancreatoduodenectomy. During operation, we found severe varices in porta hepatis which needed experienced operative skills. The whole surgery lasted for 9 hours with the volume of hemorrhage of 100ml and plasma infusion of 600ml, This patient recovered well and no complications happened until now.
