AIMS65评分系统对上消化道出血评估作用的临床价值研究

摘　　要：目的 通过回顾性分析AIMS65评分系统对上消化道出血死亡率、输血率、再出血率的评估作用,探讨其临床应用价值。方法 收集我院2011年1月~2015年12月上消化道出血病例567例,用AIMS65系统评分,按分值（0~5分）,分为6组,比较各组间死亡率、输血率、再出血率;应用受试者工作曲线（ROC）曲线,计算曲线下面积（AUC）,探讨该评分系统对预测上消化道出血的死亡、输血、再出血情况的评估作用。结果 1~6组患者死亡率分别为0.0%、2.9%、12.7%、29.2%、42.9%、94.4%;输血率分别为9.3%、17.1%、27.1%、44.1%、50.0%、72.2%;再出血率分别为3.3%、6.4%、5.4%、11.8%、13.3%、22.2%。死亡率、输血率及再出血率ROC曲线下的AUC分别为0.888、0.723、0.649。当AUC≥0.7时,评分系统诊断价值良好。结论 AIMS65评分系统对上消化道出血的死亡率、输血率有良好的诊断价值,对再出血率诊断价值较低,该评分项目简单,便于临床收集,有很好的临床应用价值。Objective To explore the clinical predictive value of AIMS65 scoring system in upper gastrointestinal hem- orrhage through retrospective analysis its mortality,rate of blood transfusion,rebleeding rate.Methods Clinical data of 567 cases of upper gastrointestinal hemorrhage admitted into our hospital from January 2011 to December 2015 were retrospectively analyzed.According to the score （0 to 5 points）,which were divided into 6 groups,mortality,rate of blood transfusion,rebleeding rate among groups were compared.The ROC curve was used to calculate the area under the curve （A UC） to evaluate the value of the scoring system in predicting the death,blood transfusion and rebleeding of upper gastrointestinal hemotThage.Results The mortality of 1-6 groups was 0.0%,2.9%,12.7%,29.2%,42.9% and 94.4% re- spectively;rate of blood transfusion rate was 9.3%,17.1%,27.1%,44.1%,50.0%,72.2% respectively;rebleeding rate was 3.3%,6.4%,5.4%,11.8%,13.3%,22.2% respectively.The A UC of mortality,rate of blood transfusion,rebleeding rate was 0.888,0.723 and 0.649 respectively.When A UC ~〉0.7,scoring system had good diagnostic value.Conclusion AIMS65 scoring system for upper gastrointestinal bleeding mortality,blood transfusion rate has a good diagnostic value,the re- bleeding rate of diagnosis is low,the score is simple,easy to clinical collection,has a good clinical value.
