摘　　要：目的 探讨超低位直肠癌保肛手术对患者预后疗效与生活质量的改善效果。方法 选取本院2010年9月-2012年9月收治的超低位直肠癌患者146例,根据其临床手术治疗方案将其分为对照组69例与观察组77例,对照组患者采用经腹会阴联合切除术（Miles术）,观察组患者给予直肠癌保肛手术。比较两组患者的术后排便功能恢复时间、术后并发症发生率、肿瘤复发率、生存状况以及生活质量评分。结果 观察组患者术后排便功能恢复时间及肛周湿疹发生率均显著低于对照组,其术后心理功能、躯体功能、物质生活状态与社会功能等生活质量评分均显著高于对照组,差异有统计学意义（P〈0.05）。结论 超低位直肠癌患者行保肛手术治疗可获得满意的预后效果,更低的术后并发症发生率,且可以积极提升患者的生活质量。Objective To investigate the improving effects of anal sphincter preserving operation for ultra-low rectal carcinoma on patients＂ prognosis curative effects and quality of life. Methods 146 patients with ultra-low rectal cancer who were admitted to our hospital from September 2010 to September 2012 were selected. According to its clinical surgi＇cal protocol, they were divided into the control group of 69 cases and the observation group of 77 cases. The patients in the control group were given abdominoperineal resection（Miles）. The patients in the observation group were given anal sphincter preserving operation for rectal carcinoma. The postoperative recovery time of defecation function, the in- cidence of postoperative complications, tumor recurrence rate; survival conditions and quality of life were compared in the two groups, and the data were analyzed statistically. Results The recovery time of defecation and the incidence rate of perianal eczema after the surgery in the observation group were significantly lower than those in the control group, and the scores of life quality, such as psychological function, physical function, material life status and social function, were significantly higher than those in the control group, the differences were statistically significant（P〈0.05）. Conclusion Anal sphincter preserving operation can achieve satisfactory prognosis, lower incidence of postoperative complications and a positive quality of life in the patients with ultra-low rectal carcinoma.
