两种结膜移植术治疗复发性翼状胬肉的对比研究

摘　　要：目的 对比研究单纯自体球结膜移植术和带自体角膜缘干细胞的球结膜移植术治疗复发性翼状胬肉的疗效。方法 选取2014年1月-2015年1月在我院就诊的复发性翼状胬肉患者70例,70眼,随机分为A、B两组,A组行翼状胬肉切除联合单纯自体球结膜移植术;B组行翼状胬肉切除联合带自体角膜缘干细胞的球结膜移植术。比较A、B两组患者术后1年的复发率。结果 A组35眼,复发3眼,复发率8.6%;B组35眼,复发2眼,复发率5.7%。术后无结膜及角膜感染、无移植片脱落及睑球粘连等严重并发症发生。结论 单纯自体球结膜移植术和带自体角膜缘干细胞的球结膜移植术在治疗复发性翼状胬肉的手术中,其两组疗效比较无统计学差异,复发率均较低,是治疗单纯翼状胬肉切除术后的复发性翼状胬肉的理想术式。Objective To compare the efficacy of autologous bulbar conjunctival autograft and autograft limbal stem cell conjunctival transplantation in the treatment of recurrent pterygium. Methods A total of 70 patients（70 eyes） with recurrent pterygium who were treated in our hospital from January 2014 to January 2015 were randomly divided into group A and group B. Group A was given pterygium excision combined with autologous bulbar conjunctival transplantation; group B was given pterygium excision combined with autograft limbal stem cell conjunctival transplantation. The recurrence rate was compared between the two groups after one year of surgery. Resulis The recurrence rate was 8.6% in 35 eyes of group A, with 3 eyes of recurrence. The recurrence rate was 5.7% in 35 eyes of group B, with 2 eyes of recurrence. No Severe complications were observed after the surgery such as conjunctival and corneal infection, graft loss and symbtepffaron. Conclusion There is no statistically significant difference of the curative effects between the two groups in the surgery of autologous bulbar conjunctival autograft and autograft limbal stem cell conjunctival transplantation in the treatment of recurrent pterygium, and the recurrence rate is low, which is the ideal surgical procedure in the treatment of recurrent pterygium after pterygium excision.
