后腹腔镜不阻断肾蒂肾错构瘤切除术(附29例报告)

摘　　要：目的探讨经腹膜外途径不阻断肾蒂腹腔镜肾错构瘤切除术的可行性与有效性。方法对我科29例行后腹腔镜不阻断肾蒂肾血管平滑肌脂肪瘤切除术临床资料进行回顾性分析。结果 29例患者手术均取得满意效果,无一例中转开放手术,术中出血少,手术时间及术后住院时间短,随访3-6个月未见肿瘤复发,肾功能均处于正常范围。结论对于肿瘤直径较小、位于肾周、未侵犯集合系统的肾血管平滑肌脂肪瘤可考虑行后腹腔镜不阻断肾蒂肾血管平滑肌脂肪瘤切除术,手术效果及患者愈后较好。Objective To explore the feasibility and effectiveness of unclamped retroperitoneal laparoscopic renal partial nephrectomy for the treatment of renal angiomyolipoma. Methods The 29 patients who underwent off-clamp retroperitoneoscopic renal angiomyolipoma resection were retrospectively analyzed. Results All the 29 cases showed satisfactory surgical results with no conversion to open surgery,less bleeding,shorter operation time and postoperative hospital stay. No tumor recurrence was found during 3-6 months＇ follow-up. Conclusion For exophytic renal angiomyolipoma with small size in diameter,not involving the collection system can be considered for off-clamp retroperitoneal laparoscopic renal angiomyolipoma resection.
