摘　　要：目的研究人乳头瘤病毒(HPV)高危型HPV-DNA阳性宫颈上皮细胞不典型增生和低级别宫颈上皮内瘤变(Lowgrade Squamous Intraepithelial Lesion,LSIL)患者pl6、Ki-67表达情况。方法用Hybrid CaptureⅡ(HC2)或FQ-PCR体外扩增技术检测宫颈上皮细胞高危型HPV﹣DNA,同时行液基细胞学检查。对97例高危型HPV-DNA阳性且液基细胞学诊断为不典型增生(ASCUS)或CINⅠ或CINⅡ的患者,行阴道镜检查取可疑宫颈组织标本多点活检并En Vision两步法进行免疫组化标记pl6、Ki-67。结果 97例宫颈上皮细胞高危HPV-DNA阳性患者中,宫颈组织CINⅡ级38例,CINⅠ级35例,炎症和(或)ASCUS 24例。P16标记结果 :3例+++,30例++,39例+,P16阳性表达率为74.2%;Ki-67标记结果 :3例+++,30例++,30例+,Ki-67阳性表达率为64.9%。炎症和(或)ASCUS、CINⅠ、CINⅡ三者比较,P16和Ki-67的阳性率和阳性程度具有极显著性差异(P<0.01)。结论高危HPV病毒阳性宫颈上皮细胞随着病情严重程度增加pl6、Ki-67的阳性率和阳性程度均升高,pl6、Ki-67的检测对CIN的准确分级有利。Objective To study the expression of P16 and Ki-67 of patients with Human papillomavirus（HPV） type of highrisk HPV-DNA and atypical hyperp-lasia or Low-grade Squamous Intraepithelial Lesion（LSIL）. Methods Test high-risk HPVDNA positive rate of cervical epithelial cells with Hybrid Capture Ⅱ（HC2） or Fluorescent quantitative polymerase chain reaction（FQ-PCR）,and LPT fluid based cytology at the same time. For 97 abnormal patients whose high-risk HPV-DNA tested positive and liquid based cytology diagnosis of ASCUS or CINⅠor CINⅡ,with Colposcopy obtain suspicious tissue of cervical multi-spot biopsy to diagnose and En Vision two-step immunohistochemical marks pl6,Ki-67. Results In 97 patients whose high-risk HPVDNA tested positive,cervical tissue CINⅡ38 cases,CINⅠ 35 cases, inflammation or ASCUS 24 cases. Marked outcomes of P16：＋＋＋ 3 cases,＋＋ 30 cases,＋ 39 cases. The positive rate of P16 was 74.2%. Marked outcomes of Ki-67：＋＋＋ 3 cases,＋＋ 30 cases,＋ 30 cases,The positive expression of Ki-67 was 64.9%. There were extremely significant difference in the positive rate and positive degree of P16 and Ki-67 by comparing inflammation and（or） ASCUS,CINⅠ,CINⅡ. Conclusion The Positive rate and positive degree of pl6 and Ki-67 of abnormal cervical epithelial cells of high-risk HPV-DNA tested positive are rising With the increase of disease severity,Screening as early as possible,The test of pl6、Ki-67 is beneficial for Accurate classification of CIN.
