脑梗死病人血清GFAP含量变化及其与认知障碍的相关性研究

摘　　要：目的探讨脑梗死病人血清胶质纤维酸性蛋白(GFAP)含量变化,并分析其与认知障碍的相关性。方法在起病3 d、1个月、3个月分别检测162例脑梗死病人血清GFAP含量,并分析其与梗死面积、梗死部位以及与卒中后认知障碍的关系。结果起病3 d、1个月时大面积梗死组病人血清GFAP水平与其他梗死面积组比较差异有统计学意义(P<0.05);皮质+皮质下梗死组病人起病3 d、1个月时血清GFAP水平与其他梗死部位组比较差异有统计学意义(P<0.05);无认知功能障碍和重度认知功能障碍病人起病3 d和1个月时血清GFAP水平与其他认知障碍组比较差异有统计学意义(P<0.05),且起病3 d、1个月时与简明精神状态量表(MMSE)评分均存在负相关(r=-0.731,P<0.05;r=-0.490,P<0.05)。结论起病3 d和1个月时的血清GFAP与脑梗死病人梗死部位和梗死面积相关而影响病人的认知功能,但起病后3个月的血清GFAP与认知功能障碍无相关性。Objective To investigate the serum glial fibrillary acidic protein（ GFAP） level in patients with cerebral infarction（ CI）,and to analyze its correlation with cognitive impairment. Methods One hundred and sixty-two patients with CI were included in this study and their serum GFAP levels at 3 days,1 month,3 months after onset were measured,and analyzes its correlation with infarct size,myocardial infarction site and cognitive impairment after stroke. Results The serum GFAP level in large infarct size patients were significantly higher than other groups at 3 days and 1 month after onset（P〈0. 05）. Patients whose infarction at cortical and subcortical infarction patients had a higher serum GFAP level at 3 days and 1 month after onset（P〈0. 05）. The serum GFAP level in patients without cognitive impairment and with severe cognitive dysfunction had statistical significance at 3 days and 1 month after onset（P〈0. 05）. There was a negative correlation of serum GFAP at 3 days and 1 month after onset and MMSE score（r =- 0. 731,P〈0. 05;r =- 0. 490,P〈0. 05）. Conclusion The serum GFAP in patients with the onset of 3 days and 1 month of CI have relationship with infarct size,myocardial infarction site which influence patients cognitive impairment,but after the onset of 3 months the serum GFAP level and each index had no relationship.
