阿托品凝胶联合山莨菪碱滴眼液治疗虹膜睫状体炎的临床疗效及安全性

摘　　要：目的探讨阿托品凝胶联合山莨菪碱滴眼液治疗虹膜睫状体炎的临床疗效及安全性。方法选取2013年5月-2015年7月浙江医院眼科收治的虹膜睫状体炎患者174例,按随机数字表法分为对照组与研究组,各87例。对照组采用托吡卡胺扩瞳,研究组采用阿托品凝胶联合山莨菪碱滴眼液扩瞳。观察并记录2组治疗前后血清炎症细胞因子水平、血清免疫球蛋白及补体水平,并对临床疗效和不良反应发生率进行比较。结果对照组治疗有效率（85.06%）低于研究组治疗有效率（94.26%）,差异有统计学意义（P〈0.05）;与对照组比较,研究组治疗后血清肿瘤坏死因子-α（tumor necrosis factor,TNF-α）、γ-干扰素（interferon-γ,IFN-γ）、白细胞介素-23（interleukin-23,IL-23）、C反应蛋白（C-reactive protein,CRP）水平较低,治疗后血清免疫球蛋白G（immunoglobulin G,Ig G）、免疫球蛋白E（immunoglobulin E,Ig E）水平较低,血清免疫球蛋白A（immunoglobulin A,Ig A）、补体3（complement 3,C3）水平较高,差异有统计学意义（P〈0.05）;2组患者不良反应发生率差异无统计学意义。结论阿托品凝胶联合山莨菪碱滴眼液治疗虹膜睫状体炎的临床疗效确切,能够有效减轻炎症反应,改善免疫状态,且安全性较高。Objective To investigate the clinical effect and safety of atropine gel combined with anisodamine eye drops on the treatment of iridocyclitis. Methods A total of 174 cases with iridocyclitis in Zhejiang Hospital from May 2013 to July 2015 in this study were selected and divided into the control group and research group with 87 cases in each group. The patients in the control group were treated with tropicamide mydriatic,the patients in the research group were treated with atropine gel combined with anisodamine eye drops mydriatic. Serum levels of inflammatory factors,serum immunoglobulin and complement levels were observed and recorded before and after treatment,the clinical efficacy and incidence of adverse reactions were compared. Results The effective rate in the control group（ 85. 06%） was lower than the research group（ 94. 26%）,the difference was statistically significant（ P〈0. 05）; compared with the control group,serum levels of TNF-α,IFN-γ,IL-23,CRP were lower in the research group after treatment,serum levels of Ig G,Ig E were lower,and serum levels of Ig A,C3 were higher after treatment,the difference was statistically significant（ P〈0. 05）; there was no significant difference in the incidence of adverse reactions between the two groups. Conclusion The clinical effect of atropine gel combined with anisodamine eye drops in the treatment of iridocyclitis was exactly,can effectively reduce the inflammatory response,improve the immune status,and high security.
