摘  要:目的 探讨脐带血采集和处理过程中微生物污染的概率、种类,并分析脐带血收集相关因素和检测方法对培养结果的影响。方法 抽取2011~2014年脐带血分离后的血浆-红细胞悬液,分别注入需氧和厌氧培养瓶,使用BACTEC9000系列全自动血液培养系统培养5d,结果阳性者做废弃处理。对阳性标本进行病原菌鉴定,同时比较不同生产方式、采集者、采集量、采集方式、处理前时间、培养体积、延迟上机时间及培养瓶种类对微生物污染率的影响。结果 6 292份被检脐带血中阳性标本131份,污染率为2.1%。病原菌以大肠埃希菌最为常见,占23.9%,其次为凝固酶阴性葡萄球菌,占20.9%。自然分娩者脐带血标本、县市级医院采集标本、采集量低于80mL的标本阳性检出率升高,培养体积由10mL提高为20mL后阳性检出率上升,差异均有统计学意义(P〈0.05)。结论 脐带血微生物污染率与生产方式、采集者及采集量相关,增加培养体积可提高检测灵敏度。Objective To investigate the rate of microbial contamination and types of pathogenic bacteria during collection and processing of umbilical cord blood units, and to explore the effects of umbilical cord blood collection related factors and detection methods on culture results. Methods The plasma and red blood cell suspension specimens from 2011 to 2014 were collected and poured into aerobic culture flasks and anaerobic culture flasks. Microbial contamination was detected by using the BACTEC9000 automatic blood culture system for 5 d,and all the positive samples were separated and identified for microbial species. The effects of several factors, including mode of delivery, collectors, volume of sample, collection method, time before sample handling, volume of culture and kinds of culture flask,on microbial contamination were also discussed in this study. Results In the 6 292 units of umbilical cord blood tested, 131 units were contaminated by pathogenic bacteria,and the contamination rate was 2.10%. The most common pathogenic bacteria were Escherichia coli(23.9%) and coagulase negative staphylococci(20. 9 %). The positive rate was increased in specimens collected from women underwent vaginal delivery,specimens collected by collectors in county(municipal) hospitals,and specimens whose blood volume was less than 80 mL; moreover, the positive rate was ascended when the culture volume increased from 10 mL to 20 mL,there was statistically significant difference(P〈0.05). Conclusion Microbial contamination was associated with mode of delivery, collector and blood volume, the increase of culture volume could promote the sensitivity.
