淮安地区6228例女性感染人乳头瘤病毒的基因型分布

摘　　要：目的探讨淮安地区女性感染人乳头瘤病毒(HPV)及其基因亚型分布情况,为HPV感染防治和流行病学研究提供依据。方法采用核酸分子杂交法筛查6 228例淮安女性生殖道21种HPV基因亚型。结果淮安地区女性HPV总感染率为37.73%(2 350/6 228),其中单纯低危亚型感染率为4.88%(304/6 228),单纯高危亚型感染率为27.76%(1 729/6 228),混合型感染率为5.09%(317/6 228)。不同年龄段之间HPV感染率比较,差异有统计学意义(P<0.05),感染率与年龄段呈近似U形曲线。2 350例HPV阳性标本中,单重、双重、三重、四重、五重及以上基因型阳性构成比分别为66.03%(1 555例);22.76%(536例)、7.09%(167例)、2.93%(69例)和1.19%(23例);高危亚型阳性构成比前5位分别为HPV16(15.23%)、HPV52(11.79%)、HPV58(9.58%)、HPV53(7.00%)和HPV33(6.95%);低危亚型阳性构成比前3位分别为CP8304(HPV81)(7.14%)、HPV11(5.30%)、HPV6(4.31%)。结论淮安地区女性HPV感染率较高,高危亚型感染以HPV16、52和58为主,低危亚型感染以HPV81、11和6为主。感染高峰年龄段为≤25岁和≥56岁。Objective To investigate the human papillomavirus（HPV） infection prevalence and subtype distribution among women in Huaian district, in order to provide a scientific basis for intervention, prevention and epidemiological studies of HPV infection. Methods A total of 21 vaginal HPV DNA types were tested among 6,228 women using DNA flow-though hybridization genotyping technique. Results The overall HPV infection rate of involved women in Huaian district was 37.73%（2,350/6,228）. The infection rate of single lowrisk HPV subtype was 4.88%（304/6,228）, that of single high-risk HPV subtype rate was 27.76%（1,729/6,228） and the multiple-infection rate was 5.09%（317/6,228）. There were significant differences in HPV infection rates among different age groups（P〈0.05）, the paragraph of HPV infection rate in different age group presented as ＂U＂ shaped curve. Among the 2,350 HPV-positive specimens, positive infection rates of single,double, three, four and five（including more than five） genotypes were 66.03%（1,555）, 22.76%（536）, 7.09%（167）, 2.93%（69） and 1.19%（23）, respectively. The top five high-risk HPV subtypes were HPV16（15.23%）,HPV52（11.79%）, HPV58（9.58%）, HPV53（7.00%） and HPV33（6.95%）. The top three low-risk HPV sub-types were CP8304（HPV81, 7.14%）, HPV11（5.30%） and HPV6（4.31%）. Conclusions The rate of HPV infection is high in the women of Huaian district. HPV16, 52 and 58 are the most common subtypes among high-risk HPV infections, while HPV81, 11 and 6 are the most common subtypes among low-risk HPV infections. The peak infection age is ≤25 years and ≥56 years.
