不同剂量右美托咪啶复合依托咪酯对胃癌根治术后患者疼痛及感染的影响

摘　　要：目的比较不同剂量右美托咪啶复合依托咪酯对胃癌根治术患者临床疗效及术后疼痛及感染的影响。方法选取行胃癌根治术的患者78例作为研究对象,采用随机数字表法分为A、B、C 3组。A组给予依托咪酯麻醉,B组给予低剂量右美托咪啶复合依托咪酯麻醉,C组给予高剂量右美托咪啶复合依托咪酯麻醉。比较3组患者血流动力学、麻醉情况、术后疼痛与感染以及不良反应发生率。结果不同时间点,A组患者血流动力学指标变化幅度明显较B、C组大(P<0.05)。3组患者在自主呼吸恢复时间、呼之睁眼时间和拔管时间等麻醉情况方面比较均无统计学差异(P>0.05)。A组患者疼痛患者及感染以及不良反应发生率远远高于B组及C组,差异有统计学意义(P<0.05)。结论右美托咪啶复合依托咪酯对于胃癌根治术患者具有良好麻醉效果,可减少患者术后疼痛及感染,减少不良反应发生。小剂量右美托咪啶即可发挥作用,故推荐临床使用小剂量治疗方案。Objective To compare the effect of different doses of dexmedetomidine and etomidate anesthesia on postop-erative pain and infection in patients underwent gastrectomy. Methods 78 patients underwent gastrectomy were randomly divided into group A,group B and group C. Group A were given etomidate anesthesia,patients in group B were given low dose of dexme- detomidine and etomidate anesthesia, patients in group C were given high dose of dexmedetomidine and etomidate anesthesia. Then hemodynamics, anesthesia, postoperative pain and infection and adverse reaction rate of the 3 groups were observed and compared. Results The change of hemodynamics of group A was bigger than group B and group C （ P 〈 0.05 ）. Time points of pontaneous breathing recover, eye open and extubation had no statistical difference of the 3 groups （P 〉 0.05 ）. Postoperative pain, infection and adverse reaction rate of group A were higher than group B with statistically difference （ P 〈 0.05 ）. Conclusion Dexmedetomidine and etomidate has good anesthesia effect, which can reduce postoperative pain, infection and adverse reaction rate. Low dose of dexmedetomidine has good effect,which is worthy of clinical application.
