腮腺沃辛瘤43例临床分析

摘　　要：目的分析腮腺沃辛瘤临床特点,探讨诊断和手术治疗方法。方法对我科近6年来收治的经手术治疗的43例腮腺沃辛瘤的临床资料进行回顾性分析。结果男女患病比为9.75∶1,50岁以上占86%,长期吸烟者占90.70%,肿瘤位于腮腺后下极占79%,肿瘤直径<4cm占86%,双侧和多发病例占16.28%,术中送冰冻病理检查34例,诊断准确率97.06%。治疗腮腺沃辛瘤的手术方式有腮腺区域性切除术、保留面神经的腮腺浅叶切除、腮腺全叶切除术。结论腮腺沃辛瘤好发于50岁以上、有长期吸烟史的男性患者,在发病情况及临床表现方面有自身特点,治疗应根据肿瘤位置、大小不同选择手术方式,位于腮腺后下极的肿瘤采用区域性切除是理想的术式。Objective By analyzing the clinical characteristics of parotid Warthin tumor,to explore the diagnosis and surgical treatment methods of Warthin tumor in parotid gland. Method The clinical data of 43 patients with the surgery of parotid Warthin tumor in our department in recent 6 years was retrospectively analyzed. Results The rate of male to female was 9.75：1,the age over 50 years accounted for 86%, long-term smokers accounted for 90.7%,and located in the posterior and inferior quadrant of parotid gland accounted for 79%, the tumor diameter less than 4cm accounted for 86%,the bilateral and multiple cases accounted for 16.28%. 34 cases took intraoperative frozen pathological examination, while the diagnostic accuracy rate was 97.06%. The surgical treatment of Warthin tumor include regional？excision、superficial parotidectomy or all parotidectomy with facial nerve preservation. Conclusion Warthin tumor in parotid gland is usually found in old man over 50 years and with long time smoking history,it has own characteristics in the incidence and clinical manifestations,surgical treatment should be based on tumor location and size. The ideal way to treat the tumor located in the posterior and inferior quadrant of parotid gland is regional excision.
