进展期胃癌患者实施腹腔镜辅助D_2根治术效果评价

摘　　要：目的探讨腹腔镜辅助D_2根治术治疗进展期胃癌患者的临床效果。方法将130例进展期胃癌患者依据随机数字表法分为对照组和治疗组,各65例,对照组采取开腹D_2根治术,治疗组则采用腹腔镜辅助D_2根治术,比较2组患者的术中及术后指标、并发症发生率、1年内转移复发率及2年生存率等。结果治疗组患者的术中出血量、排气时间及住院时间均明显小于对照组(P<0.01);治疗组患者的并发症发生率为12%,明显低于对照组的29%(P<0.05);治疗组患者的1年转移复发率为3%,明显低于对照组的14%(P<0.05);治疗组患者的2年生存率为86%,明显高于对照组的68%(P<0.05)。结论进展期胃癌患者实施腹腔镜辅助D_2根治术,可减少术中出血量,促进患者恢复,降低并发症发生率和转移复发率,提高预后生存率。Objective To explore the clinical effect of radical excision of D2 with laparoscope on the ad- vanced gastric cancer patients. Methods One hundred and thirty cases of advanced gastric cancer patients were randomly divided into control group （65 cases） and treatment group（65 cases）. The control group was given open radical gastrectomy of D2, and the treatment group was adopted radical excision of D2 with laparoscope. The inde- xes of intraoperation and postoperation, incidence rate of complications, metastatic recurrence rate of one year and survival rate of two years were compared between the two groups. Results The indexes of hemorrhage volume of intraoperation, time of venting and length of stay in the treatment group were much lower than those of the control group （ P 〈0.01）. The incidence rate of complications in the treatment group was 12%, obviously lower than that of the control group （29%, P 〈0.05）. Metastatic recurrence rate of one year in the treatment group was 3%, lower than that of the control group （14%, P 〈0.05）. And the survival rate of two years in the treatment group was 86%, much higher than that of the control group （68%, P 〈0.05）. Conclusion For advanced gastric cancer patients, carrying out radical excision of D2 with laparoscope could decrease intraoperative volume of hemorrhage, promote recovery, cut down incidence rate of complications and metastatic recurrence rate, and raise survival rate of prognosis.
