摘　　要：目的探讨鼻咽癌放射治疗后发生放射性脑病的影响因素。方法收集江门市中心医院2007年1月至2010年1月收治的鼻咽癌放疗后放射性脑病患者的临床资料,总结其临床特点,采用单因素及多因素Logistic回归进行相关危险因素的分析,探讨影响鼻咽癌放疗后放射性脑病的独立危险因素。结果同期560例鼻咽癌放射患者共26例发生了放射性脑病,发生率4.64%（26/560）;发生放射性脑病的患者中首程放疗患者23例,中位潜伏期30个月（6-69个月）,好发部位为双侧颞叶。通过单因素分析显示年龄、射线种类、再程放疗、放化疗疗程、临床分期是影响患者放疗后放射性脑病的因素;多因素分析显示射线种类、再程放疗、放化疗疗程、临床分期属于晚期是影响放疗后放射性脑病的独立危险因素。结论鼻咽癌放射性脑病是一个多因素的结果,射线种类、再程放疗、放化疗疗程、临床分期是主要影响因素,放射性脑病影响患者预后及生存质量。Objective To investigate the effect factors of radiation encephalopathy after radiotherapy and prognosis.Methods The clinical data collected from Jiangmen Central Hospital/radiotherapy of nasopharyngeal carcinoma admitted to Jiangmen Hospital Affiliated to Sun Yat-Sen University after radiation encephalopathy patients,summarize its clinical characteristics,using univariate and multivariate Logistic regression analysis of risk factors,risk of radiation encephalopathy and to explore the influencing factors of nasopharyngeal carcinoma after radiotherapy.Results The incidence of radiation encephalopathy in 26 patients was 4.64%（ 26/560）.In the patients with radiation encephalopathy,23 patients had the first course of radiotherapy with a median latency of 30 months（ 6 to 69 months） Lateral temporal lobe.Radiation type,re-radiotherapy,radiotherapy course,clinical stage belong to late stage.The prognosis of radiation encephalopathy after radiotherapy was influenced by radiotherapy,radiotherapy,radiotherapy,radiotherapy,single-factor analysis.The independent risk factors of radiation encephalopathy after radiotherapy.Conclusion Radiation encephalopathy of nasopharyngeal carcinoma is a multifactorial outcome.Radiation type,re-radiotherapy,radiotherapy and chemotherapy,clinical stage is the main influencing factor,prognosis and quality of life of patients with radiation encephalopathy.
