摘 要:目的对比研究动脉自旋标记(ASL)和动态磁敏感对比增强灌注技术(DSC-PWI)在评价视神经脊髓炎(NMO)患者脑血流灌注中的作用。方法对经临床确诊的10例NMO患者行ASL及DSC-PWI检查,应用西门子后处理工作站软件获得14个感兴趣区(双侧对称的额叶、顶叶、颞叶、枕叶、尾状核、壳核和丘脑)的脑血流量(CBF)。应用配对t检验比较两种技术所得测量比值的差异,并用Pearson相关分析两种技术所得测量值的相关性。结果 ASL和DSC-PWI所测各感兴趣区的相对CBF比值均无显著性差异。(t=1.061,0.496,1.312,-0.342,-0.403,-0.156,0.224;P=0.329,0.637,0.238,0.744,0.708,0.881,0.830)除顶叶感兴趣区以外,两种方法所测其余各感兴趣区的CBF比值均呈显著相关性(r=0.659,P=0.000)。结论 ASL在评估NMO血流灌注方面与DSC之间有相似的敏感性,可用于临床评估视神经脊髓炎的脑血流灌注。Objeetive To compare the difference between arterial spin labeling (ASL) and the dynamic susceptibility contrast-enhanced perfusion weighted imaging (DSC-PWI) for assessing brain perfusion in neuromyelitis optica (NMO) patients. Methods ASL and DSC-PWI data were acquired from 10 eases of NMO. Cerebral blood flow (CBF) of fourteen regions of interest (ROI), including caudate nucleus, putamen, thalmnus and white matter in the frontal lobe, parietal lobe, temporal lobe and occipital lobe, were measured using siemense postprocessing station. Results There was no significant difference between the two techniques. (t =1.061, 0.496, 1.312, -0.342, -0.403, -0. 156, 0.224; P =0.329, 0.637, 0.238, 0.744, 0.708, 0. 881, 0. 830). There were significant correlations between CBF measured by ASL and that by DSC ( r =0. 659, P =0. 000), except for the ROI in parietal lobe. Conelusion Non-invasive ASL perfusion is as sensitive as DSC-PWI in evaluating NMO, and can be used in the clinical assessment.
